Abstract

목적

Brevundimonas diminuta/vesicularis에 의한 각막염 1예를 경험하였기에 국내 최초로 보고하는 바이다.

증례요약

20세 남자 환자가 내원 2일 전부터 시작된 우안 시력저하 및 눈부심을 주소로 내원하였다. 초진 시 세극등검사상 우안 각막 중심부에 1.5 × 150 mm 크기의 각막상피결손을 동반한 기질 침윤, 전반적인 각막부종 소견이 관찰되었다. 배양검사상 Brevundimonas diminuta/vesicularis가 검출되었다. 경험적 항생제 안약으로 치료 후 증상의 소실과 시력회복이 관찰되었다.

결론

Brevundimonas diminuta/vesicularis에 의한 감염각막염 1예를 우리나라 최초로 경험하였기에 기존 문헌 고찰과 함께 이를 보고하고자 한다.

Trans Abstract

Purpose

To report a case of keratitis caused by Brevundimonas diminuta/vesicularis.

Case summary

A 20-year-old man presented with decreased visual acuity and glare in his right eye from 2 days earlier. At the first visit, slit lamp examination revealed a 1.5 × 1.5 mm2-sized stromal infiltration, an edematous cornea and endothelial deposits in the center of the right eye. Microbiological examinations were performed from corneal scrapings. Treatment was started with topical eye drops (fortified cefazolin, fortified tobramycin) as empirical antibiotics. After several days, the cultures revealed Brevundimonas diminuta/vesicularis. After treatment with empirical antibiotic eye drops, the symptoms and visual acuity improved.

Conclusions

We report a case of infectious keratitis caused by Brevundimonas diminuta/vesicularis, which has never previously been reported in the Republic of Korea.

감염각막염은 임상적으로 중요한 안구감염질환 중 하나로 조기 진단 및 적절한 치료가 이뤄지지 않으면 영구적인 시력저하, 실명과 같은 심각한 결과를 초래할 수 있다[1]. 전체 감염각막염 중 세균각막염은 54.0-94.2%를 차지하며,이 중 그람 음성균은 국내에서 20.1-65.3%를 차지하고 있다[2,3]. 2012년에 국내에서 세균각막염의 가장 흔한 균주는 Staphylococcus epidermis와 Pseudomonas aeruginosa로 보고된 바 있다[3].

Brevundimonas는 그람 음성균으로, 크게 프로테오 박테리아에 속하며 호기성 세균으로 알려져 있다. 과거에는 Pseudomonas 속에 속했지만 Segers at al [4]에 의해 Brevundimonas diminuta와 Brevundimonas vesicularis로 재분류되었다. Brevundimonas는 자연환경에서 흔하게 발견되며 주로 면역이 저하된 환자에서 인체감염을 일으킨다. 그러나 Brevundimonas diminuta/vesicularis가 안구에 감염을 일으켜 검출된 것은 매우 드물며, 국내에서 안감염에 대한 보고된 바가 없다. 따라서 저자들은 본 증례에서 Brevundimonas diminuta/vesicularis에 의한 각막염 1례를 국내 최초로 경험하여 이의 임상양상과 치료 경과를 문헌 고찰과 함께 보고하고자 한다.

증례보고

기저질환이 없는 20세 남자 환자가 2일 전부터 발생한 우안의 시력저하 및 눈부심을 주소로 본원 외래를 방문하였다. 환자는 내원 1주일 전 모래바람이 우안에 들어간 이후 우안 불편감이 발생했다고 진술했으며, 그 밖의 특이사항은 없었다. 최대교정시력은 우안 1/20, 좌안 20/20이었으며, 비접촉성 안압계로 측정한 안압은 우안 21 mmHg, 좌안 15 mmHg였다. 세극등검사상 양안 눈꺼풀 상태는 양호하였고, 우안의 결막은 충혈되어 있었으며, 각막 중심부에 1.5 × 1.5 mm 크기의 각막상피결손, 각막 간질의 염증 침윤 및 전반적인 각막부종이 관찰되었다(Fig. 1). 각막내피에 많은 염증세포의 각막 후 침착이 관찰되었고, 수정체, 유리체 및 안저는 각막부종으로 깨끗하게 관찰되지 않았으나 특이 소견은 없었다. 전신 혈액검사와 함께 각막찰과를 통해 검체를 채취하고 도말검사와 배양검사를 시행하였다. 검체 채취와 배양검사는 본원 초진 시 경험적 항생제를 투여하기 전에 시행하였다. 도말검사를 위해 0.5% proparacaine hydrochloride (Alcaine®, Alcon Laboratory, Fort Worth, TX, USA)로 각막을 점안마취한 뒤, No. 15 Bard-Parker knife (Bard-Parker®, Aspen Surgical, Caledonia, MI, USA)로 궤양의 가장자리와 기저 부위를 긁어서 유리 슬라이드에 도말 표본을 만들었다. 검체의 도말 후 그람 염색을 시행하였으며, 세균 및 진균 배양검사를 위해 감염 부위에서 병변이 진행하는 가장자리 부위를 면봉을 이용하여 찰과하였다. 검체를 묻힌 면봉을 이송배지에 넣어 미생물검사실로 보내고 MacConkey agar medium에 접종하였고 48시간 동안 배양을 시행하였다.

MacConkey 배지에 접종하여 배양한 결과에서 Brevundimonas diminuta/vesicularis가 중등도로 관찰되었으며, 항생제 감수성검사에서는 Ciprofloxacin, Gentamicin, Tobramycin을 포함한 대부분의 항생제에 감수성을 보였다(Fig. 2). 초진 시 각막찰과에 의한 미생물검사를 실시한 후 결과가 나오기 전에 경험적 점안 항생제로 Fortified cefazolin, Fortified tobramycin을 1시간 간격으로 투여하여 점차 횟수를 줄여나갔다. 상피의 회복을 돕기 위해 0.3% Hyaluronic acid(Hyalyuni 0.3%, Taejoon, Seoul, Korea)를 하루 4번 점안하였으며, 전방 염증 악화를 막기 위해 1% Atropine sulfate hydrate (Isopto atropine 1%, Alcon, Seoul, Korea)를 하루 2번 점안하였다. 전신 상태를 평가하기 위해 시행한 흉부 방사선 사진, 소변검사, 혈액검사는 모두 정상 소견으로 관찰되었다. 점안 7일째에는 병변의 완전 상피화와 함께 각막부종 및 각막내피 염증세포의 각막 후 침착이 소실되었다. 치료 9일째 퇴원하였으며, 퇴원 당시 우안 최대교정시력은 20/20였고, 안압은 골드만압평안압계로 측정 시 11 mmHg였다. 세극등현미경검사상 미세한 각막혼탁이 관찰되었다(Fig. 3). 이후 7일간 Fortified cefazolin, Fortified tobramycin 그리고 0.3% Hyaluronic acid을 하루 4차례 사용하였다.

고 찰

Brevundimonas diminuta/vesicularis는 프로테오 박테리아의 α-아 그룹에 속하며, 그람 음성, 호기성 세균으로 알려져 있다. 이 균은 맨 처음 1953년 Corynebacterium vesiculare라는 이름으로 Busing et al [5]에 의해 보고되었으며, 과거에는 Pseudomonas 속에 속했지만 1994년도에 DNA-rRNA 혼성화 실험과 16s rRNA 염기서열을 포함한 분자생물학적 특징을 토대로 Pseudomonas와는 독립된 종으로 밝혀져 Segers at al [4]에 의해 Pseudomonas diminuta, Pseudomonas vesicularis에서 Brevundimonas diminuta와 Brevundimonas vesicularis로 재분류되었다. DNA-rRNA hybridzation과 16S rRNA oligonucleotide와 같은 RNA 특성에 따라 Palleroni가 분류한 프로박테리아의 아형에 따르면 Pseudomonas는 Palleroni의 rRNA group I에 속하는 종이고 Brevundimonas는 Palleroni의 rRNA group IV에 속하는 종이다[4]. Brevundimonas는 물, 토양, 식물 등의 자연환경에 흔히 존재한다. 주로 혈액투석 환자, 전신적 루푸스 환자, 방실 판막수술 환자, 겸상 적혈구 빈혈증 환자 등 자가면역질환이나 오랜 기간 스테로이드 치료 과거력이 있는 면역이 저하된 환자에서 인체 감염을 일으킨다[6]. 그러나 지금까지 안구의 감염에서 Brevundimonas diminuta/vesicularis가 검출된 것은 매우 드물다. 해외에서 2010년에 라식수술 이후에 발생한 Breviundimonas vesicularis에 의한 각막염 1예[7]와 2012년에 Brevundimonas diminuta에 의한 각막염 1예[8]가 최초로 보고된 바 있으며, 아직까지 국내에서는 보고된 바가 없다. Pelletier et al [7]은 라식수술 과거력이 있는 45세 여자 환자에서 각막상피결손과 각막실질 침윤을 발견하고 시행한 배양검사상 Breviundimonas vesicularis이 동정되었다고 보고하였다. Pelletier et al [7]은 경험적 항생제로 vancomycin과 cetrazidime을 먼저 투여하고, 항생제 감수성검사 결과를 확인한 후 cefazidime과 levofloxacin으로 치료했다고 하였다. 또한 Pandit은 소프트렌즈 과거력이 있는 66세 여자 환자에서 각막궤양으로 진단하고 배양검사상 Brevundimonas diminuta가 동정되었다고 보고하였다[7,8]. 일반적으로 임상에서 세균각막염은 외상이나 소프트렌즈 착용 과거력이 있는 경우가 많지만, 본 증례에서는 세균각막염을 일으킬 수 있는 위험요인은 없었다. 또한 Brevundimonas는 주로 면역이 저하된 환자에서 기회 감염을 일으키지만, 본 증례에서의 환자는 다른 기저질환이 없었다.

세균각막염은 초기 치료로 세균배양검사 결과가 나오기 전에 광범위 항생제치료를 흔히 시행하고 있으며, 이후 배양 결과에 따라 임상적인 반응에 따라 약제 변경을 고려할 수 있다[9]. 세균각막염을 일으키는 원인균의 유병률은 보고된 연구마다 다양하다. 방글라데시의 Katz et al [10]의 연구, 대만의 Hsiao et al [11]의 연구, 1990년대 후반 국내에서 시행한 Hahn et al [12]의 연구에서는 Pseudomonas aeruginosa가 가장 많이 검출되었다. 한편 최근 10년간 서울지역 대학병원에 내원한 환자를 대상으로 한 Mun et al [13]의 세균각막염 연구에서는 Staphylococcus spp.가 28%로 가장 많았고, Pseudomonas spp.는 12.1%로 두 번째로 많이 검출되었다. 이러한 연구마다 다른 세균 분포의 차이는 환자의 기저질환이나 면역 상태, 거주지역, 직업적, 연령적 차이와 같은 요인의 차이 등에 기인할 것으로 생각된다.

지금까지 보고된 다수의 연구에서 Brevundimonas에 의한 감염의 경우 Cephalosporin, Penicillin, Aminoglycoside으로 치료하였다고 보고하였다. 그러나 Brevundimonas에 의한 감염에는 β-lactam과 Fluoroquinolone 계열과 같은 많은 항생제에 저항성이 있어, Brevundimonas에 의한 감염은 흔히 치료하는 데 어려움이 있을 수 있다. Brevundimonas에 의한 감염에 대한 정립된 치료법은 아직 있지 않아, 배양검사를 통한 항생제 감수성검사를 통한 확인이 필요하다[4]. 본 증례에서는 항생제 감수성검사상 Ciprofloxacin, Gentamicin, Tobramycin을 포함한 대부분의 항생제에 감수성을 보여 Cefazolin, Tobramycin으로 치료할 수 있었다.

본 증례에서 저자들은 임상적으로 안구 감염에서 매우 드문 Brevundimonas diminuta/vesicularis에 의한 각막염 환자를 경험하였다. 환자는 경험적 항생제로 Fortified cefazolin, Fortified tobramycin으로 치료를 시작하였으며, 치료 시작 이후 악화 소견 없이 입원치료 9일째 호전되어 퇴원할 수 있었다. Brevundimonas diminuta/vesicularis에 의한 각막염은 외국에서도 발표 사례가 드물며, 우리나라에서는 발표된 예가 없었다. 아직까지 Brevundimonas에 의한 명확한 치료 방법은 정립되어 있지 않아 앞으로 Brevundimonas에 대한 항생제 연구가 추가적으로 필요할 것으로 생각된다. 저자들은 세균각막염이 의심되는 환자에서 배양검사를 통해 Brevundimonas균을 동정하고 성공적으로 치료했던 본 증례가 앞으로 국내에서 발생한 Brevundimonas에 의한 세균각막염 환자들을 진단하고 치료하는 데 도움이 될 것으로 생각된다. 결론적으로 기저질환이 없는 젊은 남자 환자에서, 국내 최초로 Brevundimonas diminuta/vesicularis 각막염 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
