Abstract |
PURPOSE To investigate the effect of ciprofloxacin on the management of endophthalmitis, the anterior chamber irrigation or the vitrectomy with infusion fluid containing ciprofloxacin was performed. METHODS: We analyzed the records of 31 eyes of 31 patients who had been treated for endophthalmitis. Twenty cases had developed after cataract surgery, 3 cases were caused by infection of filtering bleb after trabeculectomy and 8 cases were following perforating ocular trauma. Culture and smear were performed with specimens with from conjunctival sac secretion, aqueous humor and vitreous. Ciprofloxacin(5micro gram/ml) in infusion fluid was used for anterior chamber irrigation or vitrectomy. In addition, 200micro gram/0.1ml ciprofloxacin was injected into the vitreal cavity, when vitrectomy was not performed. RESULTS: Causative organisms were identified. Staphylococcus epidermidis was cultured in 6 cases, Pseudomonas aeruginosa in 4 cases in 13 out of 31 eyes (42%), Staphylococcus aureus, viridans group streptococci, Streptococcus uberis in 1 case each. Except one case of Streptococcus uberis, all cultured organisms were sensitive to ciprofloxacin. Final visual acuity of 0.1 or better was achieved in 22 of 31 eyes after treatment with ciprofloxacin. Complications included 3 cases of retinal detachment, 2 cases of bullous keratopathy, 1 case of secondary glaucoma, 1case of optic nerve atrophy and 1 case of corneal opacity CONCLUSIONS: Our result suggests that application of ciprofloxacin by mixing in infusion fluid may be effective for the management of bacterial endophthalmitis |
