재발성 결절앞공막염에서 유리체강 내 덱사메타손 임플란트의 효과

국문초록

목적

자주 재발하는 결절성 앞공막염 환자에서 유리체강 내 덱사메타손(Ozurdex®, Allergan, Irvine, USA) 시술 후 호전된 증례를 보고하고자 한다.

증례요약

16세 여자가 반복되는 좌안 결막충혈을 주소로 내원하여, 결절성 앞공막염을 진단받고, 안약 및 경구 스테로이드로 치료 중, 고안압과 전신발진 등의 부작용으로 스테로이드를 감량한 후 재발하였다. 이에 cyclosporine (Sandimmune®, Novartis, Basel, Switzerland)과 methotrexate (Methotrexate, Korea United Pharm. INC., Seoul, Korea) 등의 면역억제제를 추가하여 치료하였으나 공막염은 호전되지 않았으며, 중간포도막염과 유리체출혈이 발생하였다. 유리체절제술을 시행한 후에도 공막염과 중간포도막염이 지속되어, 유리체강 내 오저덱스 주사를 시행하였으며, 이후 중간포도막염과 공막염이 재발없이 호전되었으며 면역억제제와 스테로이드도 중단할 수 있었다.

결론

중간포도막염 등 합병증이 동반되어 있으면서, 자주 재발되는 전부공막염에서 유리체강 내 덱사메타손 치료가 효과적일 수 있다.

ABSTRACT

Purpose

To report the use of intravitreal dexamethasone implant injection in a patient with recurrent nodular anterior scleritis.

Case summary

A 16-year-old patient presented with persistent eye pain and conjunctival injection in the left eye, which were diagnosed as nodular anterior scleritis. The clinical features improved with topical and systemic steroid treatment, but the patient developed side effects related to steroid use, including elevated intraocular pressure and skin rash. The steroid dose was reduced which led to frequent recurrence. Immunosuppressive agents were added to the treatment regimen, but there was no improvement; the patient developed intermediate uveitis and vitreous hemorrhage. Scleritis and intermediate uveitis continued despite vitrectomy; therefore, intravitreal dexamethasone implant injection was performed. There was no scleritis recurrence within 1 year after injection, which allowed tapering of steroids and immunosuppressive drugs.

Conclusions

Intravitreal dexamethasone implant injection may be effective for recurrent nodular anterior scleritis in patients who exhibit complications, such as intermediate uveitis.

공막염은 공막을 침범하는 염증성 질환으로 통증이나 충혈 등의 증상을 동반하며, 세극등검사에서 공막혈관의 확장과 공막부종이 관찰된다.1,2 공막염은 원인에 따라 감염성(infectious)과 비감염성(non-infectious)으로 그리고 침범된 공막의 위치에 따라 앞공막염(anterior scleritis)과 뒤공막염(posterior scleritis)으로 분류할 수 있다. 결절앞공막염(nodular anterior scleritis)은 비감염성 앞공막염으로 단단하면서 움직임이지 않고, 촉진 시 통증을 동반하는 결절과 국소적인 공막부종을 특징으로 한다. 일차 치료로 비스테로이드항염증제 및 전신 스테로이드를 사용할 수 있으며, 일차 치료에 반응하지 않거나 전신 스테로이드를 장기간 복용해야 할 경우에는 면역억제제를 추가적으로 사용할 수도 있으며,1 스테로이드 또는 면역억제제의 사용에도 반응을 보이지 않고 재발되는 공막염 환자에게 생물학적 제제를 사용하여 치료한 사례도 보고되고 있다.2-4
유리체강 내 덱사메타손 이식제(Ozurdex®, Allergan, Irvine, CA, USA)는 당뇨황반부종, 중심망막정맥폐쇄 혹은 분지망막정맥폐쇄에 동반된 황반부종 등에 효과적이며, 포도막염 치료에도 효과가 있다고 보고되었다.4-8 공막염 치료에서도 유리체강 내 덱사메타손 이식제는 뒤공막염 환자 그리고 류마티스 질환과 동반된 재발성 포도막염 및 공막염 환자에 시행되어 호전된 증례가 보고되기도 하였다.2,9-12 하지만 재발되는 앞공막염에서 유리체강 내 덱사메타손 삽입으로 치료한 증례 보고는 없었다. 저자들은 자주 재발되는 난치성 결절공막염 환자에서 중간포도막염 등 합병증이 동반되어 유리체강 내 덱사메타손을 투여 후 공막염이 재발되지 않고 회복된 증례를 경험하여 보고하고자 한다.

증례보고

16세 여자가 6개월 전부터 발생한 좌안 결막충혈을 주소로 내원하였다. 내원 당시 교정시력은 우안 20/20, 좌안 20/20이었고, 안압은 좌안 17 mmHg였다. 세극등현미경검사에서 좌안 결막의 충혈, 부종, 공막결절이 관찰되었으며 공막결절 부위에 압통이 있었다(Fig. 1A, B). 전방 내에 염증 소견은 없었고, 망막 및 유리체에도 이상 소견은 없었다. 좌안 결절공막염으로 진단하고, 동반된 전신 질환 감별을 위해 혈액검사 등을 시행하였다. 혈액검사상 cytoplasmic anti-neutrophilic cytoplasmic antibody가 양성이었으나, 소아청소년과에서 실시한 검사에서는 동반된 전신 질환은 없었다. 치료는 국소 스테로이드(Prednilone®, Daewoo Pharm. Co., Busan, Korea)와 경구 비스테로이드 진통소염제(Champ Syrup®, Ibuprofen, Dong-A Pharm., Seoul, Korea) 20 mL 하루 3번 및 스테로이드(prednisolone; Solondo®, Yuhan Medica, Seoul, Korea) 30 mg/day로 처방하였다. 이후 국소 및 경구 스테로이드 용량을 감량하며 경과 관찰 중 치료 2개월째 안구운동 시 통증, 결막충혈 및 혈관 확장이 관찰되어 재발성 결절공막염으로 진단하고 경구 스테로이드를 증량하였다(Fig. 1C). 이후 스테로이드 감량하여 유지 용량 복용 시 공막염이 재발되어 사이클로스포린 100 mg (Sandimmune®, Novartis, Basel, Switzerland)을 하루 2번 복용하였다. 환자는 경구 스테로이드를 15 mg/day 이하로 감량 시 반복적으로 재발하였으며, 장기간 스테로이드 사용으로 인해 안압 상승, 전신발진 등 약제 부작용이 동반되어 메토트렉세이트 7.5 mg (Methotrexate, Korea United Pharm. INC., Seoul, Korea)을 1주일에 1회로 추가하였으며, 이후 공막부종 및 혈관 확장이 호전되는 것을 확인하였다(Fig. 1D). 하지만 좌안충혈과 통증이 재발되었고 환자가 좌안 시력저하 및 비문증을 호소하였다. 좌안 시력은 20/100으로 감소되었고 좌안 안저검사상 시신경신생혈관 및 평면부 눈더미가 관찰되어, 난치성 결절공막염과 합병된 중간포도막염으로 진단하였다(Fig. 2A-C). 국소 스테로이드 점안제와 메토트렉세이트를 증량(15 mg/week)하고, 유리체강 내 베바시주맙(Avastin®, Genentech, San Francisco, CA, USA) 주입술을 2회 시행하였으나 호전되지 않았고 좌안 유리체출혈 발생하여 좌안 유리체절제술을 시행하였다. 이후 유리체출혈은 발생하지 않았으나, 결절공막염과 중간포도막염의 반복적인 재발로 인해(Fig. 3A) 사이클로스포린 100 mg 하루 한번, 메토트렉세이트 15 mg을 1주일에 1회 유지하면서 1년 간격으로 유리체강 내 덱사메타손 임플란트 0.7 mg을 2회 투여하였다. 마지막으로 유리체강 내 덱사메타손 임플란트를 투여하고 1년간 경과 관찰하였으며 공막염이나 중간포도막염이 재발하지 않았고, 스테로이드와 면역억제제 사용을 중단할 수 있었다(Fig. 3B, C).

고 찰

비감염성 공막염의 일차 치료는 비스테로이드성 진통소염제과 국소 및 전신 스테로이드를 사용하는 것이다. 하지만 공막염 치료 중에 스테로이드를 고용량으로 사용해도 호전되지 않거나, 감량하는 과정에서 자주 재발하는 경우에는 면역억제제와 생물학적 제제 등을 추가적으로 치료에 이용할 수 있다.1 공막염에서 면역억제제와 스테로이드 사용에도 효과가 없었던 증례에서, de Fidelex et al3은 47명의 공막염 환자 중 infliximab 사용 후 45명(96%)이 호전되었으며, 이 중 64%의 환자에서는 스테로이드를 줄이거나 중단할 수 있었다고 보고하였다. 또한 rituximab을 사용하여 26명의 공막염 증례에서 모두 완치되거나 재발 방지 효과가 있었다고 보고하였다.3
덱사메타손 임플란트는 이식제 1개에 수용성 부신피질호르몬인 덱사메타손 700 μg을 함유하고 있으며 덱사메타손을 저용량으로 수개월 동안 서서히 방출하는 생분해성 삽입물로, 유리체강 내 트리암시놀론에 비해 더 오랜 기간 작용하면서 안압상승 등의 부작용은 적은 것으로 알려져 있다.2,13 또한 국소적으로 스테로이드 농도를 높일 수 있어 전신 스테로이드를 포함한 항염증제의 전신 사용 용량을 줄일 수 있으며, 약효는 최대 6개월 지속될 수 있다고 알려져 있다.2,14,15 포도막염 치료에 흔히 사용하며 망막정맥폐쇄, 당뇨망막병증으로 인한 황반부종 치료에도 사용한다.4-8 공막염의 치료로 덱사메타손 임플란트 삽입술은 흔하게 사용하지는 않지만, 난치성 후부공막염 또는 포도막염을 동반한 공막염 치료에 유리체강 내 덱사메타손 주입술을 시행한 증례들이 보고되었다.2,10,11,13
앞공막염 치료에서 결막하 덱사메타손 임플란트 삽입술을 사용한 증례 보고는 있으나, 유리체강 내 삽입하여 치료하였다는 증례는 아직 없다.12 본 증례의 경우 전신 질환을 동반하지 않은 소아에서 난치성 앞공막염이 중간포도막염과 동반되었고 기존의 전신 및 국소 스테로이드와 면역억제제로 치료를 하였으나 자주 재발하였으며, 약제 사용으로 인한 부작용이 발생하여 유리체강 내 덱사메타손 임플란트 삽입술을 시행하였다. 이후 공막염이 모두 호전되어 완전 관해를 이루었고 스테로이드 사용도 중단할 수 있었다. 덱사메타손 주입 후 안압상승과 백내장 등 부작용이 발생할 수 있다.5 하지만 장기간 스테로이드의 사용으로 인한 약제 부작용이 발생하거나, 기존의 치료로 호전을 보이지 않는 앞공막염에서 포도막염이 동반되어 있는 소아의 경우 유리체강 내 덱사메타손 임플란트 주입을 고려해볼 수 있을 것으로 생각된다.
본 증례를 통해 장기간 스테로이드 사용이 어렵고 기존의 치료에 반응이 떨어지는 난치성 앞공막염에서 포도막염이 동반되었을 경우 유리체강 내 덱사메타손 임플란트 투여가 효과적인 치료일 수 있음을 확인하였다. 하지만 추후 더 많은 증례와 장기간 연구를 통해 재발 앞공막염에서 유리체강 내 덱사메타손 주입에 대한 효과와 안정성에 대한 평가가 이루어져야 할 것으로 생각된다.
