ARTIS® PL E 인공수정체를 이용한 백내장수술의 임상적 결과

목적

백내장수술시 ARTIS® PL E (Cristalens Industrie, Lannion, France) 인공수정체와 Tecnis® ZCB00 (Johnson & Johnson Vision, Santa Ana, CA, USA) 인공수정체를 삽입한 환자들의 임상 결과를 비교하였다.

대상과 방법

동일한 술자로부터 백내장초음파유화술을 받고 ARTIS® PL E를 삽입한 33안(A군)과 Tecnis® ZCB00을 삽입한 45안(B군)의 의무기록을 후향적으로 분석하였다. 수술 후 1, 3개월 되는 시점에서의 최대교정시력, 굴절력 및 고위수차를 수술 전과 비교하였다.

결과

수술 전 최대교정시력은 A군과 B군 간 유의한 차이는 없었다(p=0.494). 수술 후 최대교정시력은 각각 logMAR 0.01 ± 0.05, 0.01 ± 0.04로 두 군 모두 유의미한 호전을 보였으나(p<0.001), 두 군 간 유의한 차이는 없었다(p=0.289, p=0.471). 수술 후 1, 3개월 경과한 시점에서 전체, 구면, 코마수차는 모두 수술 전과 비교하여 유의하게 감소하였다(p<0.05). 두 집단 모두 수술 1, 3개월 후 목표 굴절력 ± 0.50 diopters 이내가 확인되어 예측도 면에서 유의한 차이는 없었다(p=0.390, p=0.959). 또한, 각 그룹에서 수술 후 1, 3개월째 굴절력에는 통계학적으로 유의한 변화는 없었다(p=0.482, p=0.372).

결론

ARTIS® PL E 인공수정체를 이용한 백내장수술 후 최대교정시력의 유의미한 향상을 보였으며, 수술 후 시력 변화 및 고위수차에서 기존에 임상효과가 검증되었던 Tecnis® ZCB00과 대등한 결과를 보였다.

ABSTRACT

Purpose

To compare the clinical outcomes of cataract surgery using the ARTIS® PL E (Cristalens Industrie, Lannion, France) intraocular lens (IOL) and conventional Tecnis® ZCB00 (Johnson & Johnson Vision, Santa Ana, CA, USA) IOL.

Methods

This retrospective study examined patients who underwent in-the-bag implantation of either an ARTIS® PL E (33 eyes, group A) or Tecnis® ZCB00 (45 eyes, group B) IOL after phacoemulsification performed by a single surgeon. Best-corrected visual acuity (BCVA), spherical equivalent, and higher-order aberrations (HOA) were measured 1 and 3 months after cataract surgery.

Results

Preoperative BCVA did not differ significantly in groups A and B. Postoperative BCVA at 1 and 3 months improved significantly (p < 0.001) in both groups compared to preoperative baseline BCVA. At 1 and 3 months postoperatively, total HOA, spherical aberration, and coma were significantly lower compared to the preoperative baseline HOA (p < 0.05) in both groups. However, there were no significant differences in the trefoil values 1 and 3 months postoperatively compared to the preoperative baseline in both groups. The absolute refractive error 3 months postoperatively was 0.27 ± 0.20 (group A) and 0.28 ± 0.20 (group B), both within ± 0.50 diopters of the targeted goal diopter; there were no significant differences in the accuracy or predictability of the IOL power calculation in both groups (p = 0.390, p = 0.959). The absolute refractive error 1 and 3 months postoperatively did not differ significantly; there were no significant differences in the stability of both IOLs (p = 0.482, p = 0.372).

Conclusions

Conventional cataract surgery using the ARTIS® PL E IOL significantly increased the BCVA, while obtaining comparable clinical results to the verified Tecnis® ZCB00 IOL in postoperative visual acuity and HOA.

1967년 처음 시도된 초음파유화술을 이용한 백내장수술은 현재 가장 일반적으로 사용되는 백내장수술 방법으로, 낭내 혹은 낭외 백내장적출술을 시행하던 이전 방식에 비해 필요한 절개창의 크기 및 수술 시간을 효과적으로 줄일 수 있게 되었다. 또한 수술 후 최종 시력, 회복 속도, 합병 증의 발생 비율 등의 여러 가지 측면에서 우수한 결과를 보여 아직까지 백내장수술의 대표적인 방법으로 이용되고 있다[1-3].
백내장수술 시 사용되는 인공수정체는 형태 및 재질에 따라 각각 다른 광학적인 특성 및 생체 적합성을 가지고 있어 수술 후 시력 및 합병증 발생에 있어 차이를 보일 수 있다는 것이 잘 알려져 있다[4]. 따라서 다양한 재질과 디자인의 인공수정체가 지속적으로 개발되어 왔으며, 새로운 인공수정체의 연구 및 개발은 계속되고 있다.
인공수정체의 재질은 예전의 polymethyl methacrylate 및 실리콘에서 발전하여 최근에는 소수아크릴 제제를 사용하는 추세이며, 이는 염증반응을 포함한 전반적 생체 적합성 면에서 기존 인공수정체에 비해 월등한 임상 결과를 보여 주고 있다[5]. 인공수정체의 형태 역시 기존 구면 인공수정체에서 비구면 인공수정체로 발전하였는데, 각막의 양의 구면수차를 줄이기 위해 설계된 비구면 인공수정체는 음의 구면수차를 갖고 있어 여러 연구에서 우수한 대비 감도 및 향상된 고위수차 측정치가 보고되었다[6,7]. 또한, 접힘 인공 수정체 디자인은 절개창의 크기를 작게 하는 데에 큰 기여를 하였으며, 수술 후 염증의 감소, 절개창으로 인한 난시의 감소, 빠른 회복 등 임상적인 장점들을 보여주고 있다[8-11].
기존에 널리 사용되고 있는 Tecnis® ZCB00 (Johnson & Johnson Vision, Santa Ana, CA, USA) 인공수정체는 음의 구면수차 -0.27 μm를 갖고 있는 비구면 단초점인공수정체 이다. 술자가 직접 주입기에 장착 후 후방에 삽입하는데, 주입기에 인공수정체를 장착하는 과정에서의 감염 위험성[12-14] 및 삽입 속도에 따른 절개창 변형의 가능성[15]에 대한 대안으로 수술 시 바로 사용할 수 있도록 주입기에 장착된 인공수정체가 설계되었으나, 새롭게 개발된 제품들이 기존 인공수정체와 비슷한 임상 결과를 보이는지에 대해서는 아직 보고된 바가 적다[16].
ARTIS® PL E (Cristalens Industrie, Lannion, France) 인공수정체는 일회용 인공수정체 주입기에 미리 장전된 상태로 개발된 새로운 일체형 비구면 인공수정체로, 네 개 고리 모양의 지지부와 6 mm 크기의 광학부로 이루어져 있으며 소수아크릴 재질로 만들어진 인공수정체이다. 2.0 mm 크기의 미세각막절개로 안내 삽입이 가능하도록 만들어졌으며, 음의 구면수차 -0.10 μm를 가지도록 설계되었다. 본 연구에서는 주입기에 미리 장착되어 있는 preloaded 인공수정체인 ARTIS® PL E와 기존에 널리 보급되어 여러 임상 연구에서 안정성이 입증된 인공수정체인 Tecnis® ZCB00의수술 후 임상 결과를 수술 후 시력 및 고위수차 변화 측면에서 비교해 보고자 하였다.

대상과 방법

2019년 6월부터 2019년 12월까지 연세대학교 의과대학 세브란스병원에서 한 명의 술자에게 백내장초음파유화술 및 후방인공수정체삽입술을 받고 3개월 이상 경과 관찰이 가능했던 61명의 의무기록을 후향적으로 분석하였다. ARTIS® PL E을 낭내 삽입한 29명 33안과 Tecnis® ZCB00을 낭내 삽입한 32명 45안을 대상으로 하였다.
수술 전 검사에서 각막혼탁, 녹내장 및 망막질환 등 안과적 질환이 동반되거나 외상 및 수술력이 있는 경우, 수술중 후낭파열이 발생하거나 수술 후 염증 및 인공수정체의 중심이탈 등의 합병증이 발생한 경우는 제외하였다. 또한, -6 diopter (D) 이상의 구면대응치(spherical equivalent, SE)이거나 26 mm 이상의 안축장 길이인 경우는 제외하였다. 연구에 앞서 세브란스병원 임상시험심사위원회(Institutional Review Board, IRB)와 윤리위원회의 심사를 승인(승인 번호: 2021-1618-001) 받았으며, 모든 환자는 수술 전 백내장 수술 시 발생 가능한 합병증에 대해서 충분한 설명을 듣고 동의하였다.
수술 전 검사는 양 군 모두에서 동일하게 시행되었으며, 수술 전 세극등 및 안압검사, 나안시력검사, 최대교정시력 검사, 현성굴절검사, 안저검사, 각막내피세포검사, 각막지형도검사(Pentacam HR, Oculus, Wetzlar, Germany), 안구 수차 검사(iTrace, Tracey Technology, Houston, TX, USA)를 시행하였다. Lens opacities classification system (LOCS) III의 nuclear (N), cortical (C), posterior subcapsular (P)로 분류하여 백내장 정도에 대한 비교 평가를 시행하였다. 인공수정체 도수는 IOL Master 700 (Carl Zeiss Meditec, Jena, Germany)을 측정하고 Sanders-Retzlaff-Kraff/Theoretical (SRK/T) 공식을 이용하여 계산하였다. 목표 굴절력(goal diopter)은 비교적 정시에 가장 가까운 근시값을 택하였고, 환자가 원하는 경우에는 반대안의 굴절력을 고려하여 목표 도수를 정하였다.
수술은 0.5% proparacaine hydrochloride (Alcaine, Alcon laboratories, Fort Worth, TX, USA) 및 4% lidocaine 점안 마취하에 시행되었다. 2.8 mm 크기의 투명각막절개창을 각막 이측 윤부에 만들어 앞방 내에 점탄물질 주입 후 26게이지 주사 침을 사용하여 5.0-5.5 mm의 전낭원형절개를 시행하였다. 수력분리술 및 수력분층술 시행 후 초음파(Centurion Vision System, Alcon, Fort Worth, TX, USA)를 이용한 수정체유화술로 핵을 제거하였으며, 낭내 잔류수정체 겉질은 관류흡입기를 이용하여 완전히 제거하였다. 점탄물질을 전방 내 주입 후 ARTIS® PL E 혹은 TECNIS® ZCB00 인공 수정체를 낭내에 삽입하였으며, 관류 및 흡인을 시행하여 남은 점탄물질을 제거하였다. 수술절개창은 봉합하지 않고 평형염액으로 부종을 일으켜 자체 폐쇄되도록 하였다.
수술 후 검사는 양 군 모두에서 동일하게 시행되었으며, 수술 후 1, 3개월째에 현성굴절검사로 최대교정시력을 측정하여 minimum angle of resolution (logMAR)로 환산하였으며, 구면대응치를 측정하였다. 수술 전 목표한 굴절력과 수술 후 1, 3개월의 구면대응치의 차이를 계산하여 비교하 였다. 또한, 수술 후 1, 3개월째에 iTrace 수차계를 사용하여 눈 전체의 수차(ocular aberration)를 측정하였다. 측정된 수차는 Zernike 상수로 표시하였으며, 구면수차(spherical aberration), 코마수차(coma aberration), 트레포일 수차(trefoil aberration) 및 총 고위수차의 제곱 평균 제곱근(root mean square of total higher order aberrations)을 분석하였다. 통계학적 분석은 IBM SPSS software ver. 26.0 (IBM Corp., Armonk, NY, USA)을 이용하였고, student’s t-test와 chi-square test를 이용하여 p값이 0.05 미만인 경우를 통계학적으로 유의하다고 정의하였다.

결과

총 61명의 78안을 등록하였는데, 그중 ARTIS® PL E이 29명 33안, Tecnis® ZCB00이 32명 45안에서 삽입되었다. ARTIS® PL E와 Tecnis® ZCB00 모두 일체형 비구면 소수 아크릴인공수정체로, 본 연구에서 사용된 도수의 인공수정체는 광학부 크기가 6.0 mm로 모두 동일하였다. 반면, ARTIS® PL E의 경우 광학부와 지지부를 포함한 전체 길이가 10.79 mm, Tecnis® ZCB00의 경우 전체 길이가 13.0 mm 였으며, ARTIS® PL E의 지지부 개수는 4개, Tecnis® ZCB00의 지지부 개수는 2개로 상이하였다(Table 1, Fig. 1).
ARTIS® PL E를 삽입한 29명의 평균 연령은 67.97 ± 10.45세였으며 Tecnis® ZCB00를 삽입한 32명의 평균 연령은 70.47 ± 7.73세로 양 군 간 유의한 차이가 없었다(p=0.229). 수술 전 최대교정시력(logMAR)은 각각 0.36 ± 0.37, 0.31 ± 0.29로 두 집단 간 유의한 차이는 보이지 않았다(p=0.494). 수술 전에 측정한 안축장 길이는 ARTIS® PL E군에서 23.58 ± 0.94 mm, Tecnis® ZCB00군에서 23.49 ± 0.73 mm 였으며(p=0.620), 양 군에서 삽입한 인공수정체 도수는 각각 21.40 ± 2.60 D, 21.40 ± 1.84 D로 두 그룹 간 통계적으로 유의한 차이가 없었다(p=0.995) (Table 2).
수술 후 1, 3개월 뒤의 최대교정시력(logMAR)은 ARTIS® PL E군에서 각각 0.01 ± 0.04, 0.01 ± 0.05, Tecnis® ZCB00 군에서 0.02 ± 0.05, 0.01 ± 0.04로 측정되어 두 집단 모두 수술 전과 비교하여 수술 후 유의미한 최대교정시력의 호전을 보였으나(p<0.001), 두 군 간의 유의한 차이는 보이지 않았다(p=0.289, p=0.471). 또한, 수술 후 1개월째 시력과 수술 후 3개월째 시력을 비교하였을 때 통계학적으로 유의한 변화는 없었다(p=0.610, p=0.596) (Table 3).
수술 전 목표한 굴절력은 안축장 길이가 22.5 mm 이상의 경우 SRK/T 공식을, 22.5 mm 미만인 경우 Hoffer Q 공식을 이용하였으며 ARTIS® PL E군에서 -0.47 ± 0.74 D, Tecnis® ZCB00군에서 -0.22 ± 0.18 D로 ARTIS® PL E군에서 더 근시에 맞추도록 목표한 것으로 확인되었다(p=0.013). 단, 수술 전 목표한 굴절력과 수술 후 1, 3개월의 구면대응치의 차이를 절대값으로 환산하였을 때 ARTIS® PL E군에서 각각 0.27 ± 0.19 D, 0.27 ± 0.20 D, Tecnis® ZCB00군에서 0.31 ± 0.22 D, 0.28 ± 0.20 D로 측정되어 두 집단 모두 수술 후 1, 3개월 후 예측도 면에서 목표 굴절력 ±0.50 D 이내가 확인되었으며, 두 군 간의 통계학적으로 유의한 차이는 없었다(p=0.390, p=0.959). 또한, 수술 후 1개월째 굴절력과 수술 후 3개월째 굴절력을 비교하였을 때 통계학적으로 유의한 변화는 없었다(p=0.482, p=0.372) (Table 4).
연구에 포함된 모든 환자의 모든 안에서 수술 전, 수술후 1개월, 수술 후 3개월 고위수차 측정이 가능하였다. 수술 후 1개월 경과한 시점에서 iTrace를 이용하여 측정한 고위 수차 중 안구 전체 수차, 구면수차 및 코마수차는 ARTIS® PL E군과 Tecnis® ZCB00군에서 수술 전과 비교하여 통계적으로 유의한 감소 소견이 확인되었다(p<0.05). 단, 트레포일수차의 경우 두 군 모두에서 수술 전과 비교하였을 때 유의한 감소 소견이 확인되지 않았다(p>0.05). 수술 후 3개월 경과한 시점에서도 마찬가지로 안구 전체 수차, 구면수차 및 코마수차는 ARTIS® PL E군과 Tecnis® ZCB00군에서 수술 전과 비교하여 유의한 감소 소견이 확인되었다 (p<0.05). 단, 트레포일수차의 경우 두 군 모두에서 수술 전과 비교하였을 때 유의한 감소 소견이 확인되지 않았다 (p>0.05) (Table 5).
전체 수차, 구면수차, 코마수차 및 트레포일수차를 포함한 모든 고위수차에서 시점과 관계없이 ARTIS® PL E군과 Tecnis® ZCB00군 두 군 간의 유의한 차이는 보이지 않았다(p>0.05). 또한, 수술 후 1개월째 고위수차와 수술 후 3개월째 고위수차를 비교하였을 때 모든 경우에서 통계학적으로 유의한 변화는 없었다(p>0.05) (Table 5).
인공수정체를 삽입한 78안 모두에서 3개월 간의 추적 관찰 도중 독성전방증후군, 인공수정체의 중심이탈, 안내염, 안압상승, 홍채후유착 등의 합병증은 관찰되지 않았다. 그 외에도 야간 눈부심, 빛번짐 등의 이상광시증 등의 지속으로 인공수정체를 교체한 사례는 없었다.

고찰

본 연구는 ARTIS® PL E 인공수정체의 임상성적 보고에 의의가 있다. ARTIS® PL E와 Tecnis® ZCB00은 모두 일체형 소수아크릴인공수정체라는 공통점이 있지만, 광학부 지름과 지지부를 포함한 전체 인공수정체 길이, 구면수차 값, 지지부의 개수와 디자인, 미세각막절개창 크기, 그리고 주입기 장착 여부 등에서 상이한 특징을 갖고 있다. ARTIS® PL E 인공수정체는 기존의 Tecnis® ZCB00 인공수정체에 비해 완전히 펼쳐지는 데에 필요한 시간이 더 짧으며, 주입시 필요한 압력이 더 낮지만 아직까지 임상 결과에 대해서는 국내 및 국제적 보고가 없었다. 본 연구는 새로 소개된 ARTIS® PL E 인공수정체와 기존에 널리 보급되어 여러 임상 연구에서 안정성이 입증된 Tecnis® ZCB00 인공수정체의 임상 결과를 비교하고자 하였다.
나안시력 및 최대교정시력에 대해 3개월간 경과 관찰한 결과, 두 군 모두에서 수술 전과 비교하였을 때 유의한 호전 소견이 확인되었다. 구면렌즈 대응치 면에서도 수술 전과 비교하였을 때 통계적으로 유의한 소견이 확인되었다. 예측도 면에서는 ARTIS® PL E 인공수정체를 삽입한 모든 환자에서 목표 굴절력 ± 0.50 D 이내의 도수 정확성이 확인되어 기존의 Tecnis® ZCB00 인공수정체와 비교하였을때[17-21] 우수한 예측도를 보여 도수 정확도 면에서 대등한 것을 알 수 있었다. 장기적으로 보았을 때 인공수정체의 굴절 안정성에는 광학부의 전후방 움직임이 중요한 요소로 작용하는데, 광학부 테두리 모양, 지지부 재질 및 모양, 그리고 광학부와 지지부가 이루는 경사[22-24] 등이 영향을 미칠 수 있으며 인공수정체의 전방이동 또는 후방이동으로 굴절력이 근시 또는 원시로 이행[25,26]할 수 있음이 보고되었다. 본 연구에서 경과 관찰하였던 수술 후 3개월까지는 ARTIS® PL E 및 Tecnis® ZCB00 인공수정체를 삽입한 환자에서 유의미한 굴절력의 변화가 관찰되지 않았다. 본 연구에서 사용된 두 가지 인공수정체 모두 일체형으로, 안정적인 고리 모양의 지지부가 수술 후 굴절 안정성에 일정 부분 영향을 미쳤을 것으로 생각된다.
고위수차 측면에서도 기존에 입증된 Tecnis® ZCB00과 비교하였을 때 대등함을 알 수 있었다. 수술 후 1, 3개월째두 군의 전체 고위수차가 통계적으로 유의하게 감소하였고, 두 가지 인공수정체가 모두 수술 후 전체 안구의 구면수차를 줄여주는 음의 구면수차를 가진 비구면 인공수정체이기 때문에 구면수차의 측정값이 낮게 유지되는 것을 확인할수 있었다. 이전에는 시력향상만을 목표로 하였다면, 점차 고위수차를 줄여 대비감도를 개선시키는 기능적 시력 (functional vision) [27]이 중요시되고 있다. 특히 시력의 질에 영향을 주는 것으로 알려져 있는 수차(코마, 트레포일, 고위수차 총합)는 두 그룹 간 유의미한 차이를 보이지 않았으며, 이를 통해 새롭게 개발된 ARTIS® PL E 인공수정체와 Tecnis® ZCB00 인공수정체가 수술 후 수차 면에서 대등함을 확인할 수 있었다.
본 연구에서 사용된 ARTIS® PL E 인공수정체는 디자인 측면에서 여러 가지 우수한 특성을 가지고 있다. 첫째, ARTIS® PL E 인공수정체가 미리 삽입기에 장착된 상태로 제작된 preloaded type의 인공수정체로 수술방에서 술자가 삽입기에 인공수정체를 장착하는 과정을 생략함으로써 삽입 시간을 단축[28]시키고, 장착 중 발생하는 오염을 줄인다는 것이다. 장착형 인공수정체의 주입구로 인해 감염의 위험성이 높아질 우려가 있다는 보고[29]도 있었으나 대체로 소독 후 재사용하던 기존의 삽입기와 달리 한 번의 수술 후 폐기하는 일회용 장착기는 도구 재사용으로 인한 오염을 줄일 수 있다는 의견이 지배적이다. 본 연구에서는 한 명의 술자로부터 동일한 환경에서 수술을 받은 환자들을 비교하기 위해 Tecnis® ZCB00을 사용한 환자들을 대상으로 하였으나 동일한 광학적 특성의 preloaded type인 Tecnis® PCB00 (Johnson & Johnson Vision, Santa Ana, CA, USA) 인공수정체도 시판되고 있어 이를 비교하는 연구도 필요할 것으로 생각된다. 둘째, ARTIS® PL E 삽입기 입구의 직경이 2.0 mm로 작아 미세각막절개수술에 적합하며 빠른 회복이 가능하다는 것이다. 본 연구에서는 두 군 모두 동일 술자에 의해 동일한 수술 방법으로 진행되었으며 수술 시 2.8 mm 크기의 투명각막절개창이 이용되었으나, ARTIS® PL E 인공수정체는 이보다 작은 크기의 절개창으로도 인공수정체 삽입이 가능하므로 더 작은 크기의 절개창으로 수술한 경우에 대한 연구가 진행된다면 추가적인 장점 및 효과를 입증할 수 있을 것으로 생각된다. 셋째, ARTIS® PL E 인공수정체는 기존 Tecnis® ZCB00과 비교하였을 때 낮은 도수(<5.0 D)부터 높은 도수(>34.0 D)까지 더 넓은 범위의 인공수정체가 생산되어 다양한 환자군에서 유용하게 사용될 수 있을 것으로 생각된다. 단, 본 연구에서는 안축장이 대부분 평균적인 범위에 속하고 있어 고도근시나 고도원시와 같이 정상 범위에서 많이 벗어난 백내장환자군을 대상으로 높거나 낮은 도수의 인공수정체를 사용한 증례가 없어 이에 대한 연구가 추가로 필요할 것으로 생각된다.
본 연구는 제한적인 대상자 수에 대해 후향적으로 진행된 연구로, 더 많은 환자를 대상으로 한 전향적 연구 결과가 필요할 것으로 생각된다. 또한, 새로운 인공수정체의 임상 결과를 분석하는 여러 연구에서 수술 3개월 후 결과값과 수술 6개월 및 1년 후 결과값에 유의미한 차이는 없는것[30-32]으로 보고되고 있으나, 더 장기적인 안정성 입증을 위해 장기간 추적 관찰한 연구가 도움이 될 것으로 생각된다. 결론적으로 국내외 연구에서 ARTIS® PL E 인공수정체의 임상 결과에 대해 보고한 것은 본 연구가 처음이며, 기존에 임상효과가 검증된 Tecnis® ZCB00 인공수정체와 비교하여 수술 후 1, 3개월의 시력 및 고위수차에서 유사하게 우수한 임상적 결과를 보임을 확인하였다.
