Abstract

목적

하사근을 침범한 안와의 phakomatous choristoma 1예를 보고하고자 한다.

증례요약

2개월 남아가 출생 시부터 있었던 우측 안와 종괴를 주소로 내원하였다. 안와 자기공명영상에서 우측 안와의 하내측에 균일하게 조영증강되는 경계가 명확한 종양이 관찰되었고, 골미란 소견은 없었다. 이에 결막 경유 안와절개술을 시행하였고, 하사근을 침범한 안와종양을 확인한 후 절제하였다. 조직병리검사 결과 콜라겐 기질 내에 상피세포로 구성된 거짓샘 구조(pseudo-glandular structure)와 이를 둘러싸는 두꺼운 기저막, 기질 내의 일부 사종형 석회화(psammo-matous calcific foci)와 관내강의 호산성 물질이 관찰되었다. 면역조직화학검사상 S-100, 사이토 케라틴(cytokeratin) 양성 소견을 보여 phakomatous choristoma로 진단하였다.

결론

본 증례는 하사근을 침범한 안와의 phakomatous choristoma를 확인한 국내 첫 보고이다. Phakomatous choristoma의 발생 빈도는 드물지만, 안와의 하내측에 발생한 종괴를 감별진단할 때 고려하여야 한다.

Purpose

We report a case of phakomatous choristoma presenting as an orbital tumor with involvement of the inferior oblique muscle.

Case summary

A 2-month-old male infant presented to our clinic with a right orbital mass that had been present since birth. Magnetic resonance imaging demonstrated a homogenous enhanced well-defined mass located in the inferomedial portion of the right orbit without bone erosion. By transconjunctival orbitotomy, the orbital tumor invading the inferior oblique muscle was identified and resected. Histopathology showed a thick basement membrane surrounding pseudoglandular structures embedded in a collagenous stroma, psammomatous calcific foci in the stroma, and eosinophilic material in the lumen. Immunohistochemical analysis showed positive staining for S-100 and cytokeratin. On histopathological evaluation, the tumor was diagnosed as phakomatous choristoma.

Conclusions

To our knowledge, this is the first report in South Korea of phakomatous choristoma of the orbit with involvement of the inferior oblique muscle. Although rare, phakomatous choristoma should be included in the differential diagnosis of tumors occurring on the inferomedial side of the orbit.

Phakomatous choristoma는 1971년 Zimmerman [1]이 처음 기술한 수정체 기원의 희귀한 종양이다. 이 종양은 매우 드물게 발생하고, 일반적으로 유피낭종 또는 기타 부속기 종양으로 오인된다. 현재까지 27예가 보고되었으며[2] 주로 영아에서 눈꺼풀 하내측의 유동성 종괴로 나타난다. 종양이 안와를 침범하거나 완전히 안와 내에 위치한 몇몇 증례도 보고되었고, 한 예에서는 각막 기질에 발생하였다[3]. 비특이적인 임상양상과 영상검사 결과를 보이기 때문에 수술 후 조직병리학으로 진단한다.

해외에서는 외안근을 침범한 phakomatous choristoma에 대해 Romano et al [4], Harris et al [5]이 보고한 적 있으나, 국내에서는 Shin et al [6]이 보고한 눈꺼풀에 발생한 증례 이외에 외안근을 침범한 경우에 대해서는 보고된 바가 없었다. 이에 본 저자들은 하사근을 침범한 안와의 phakomatous choristoma 1예에 대하여 보고하는 바이다.

증례보고

2개월 남아가 출생 시부터 있었던 우측 안와 종괴를 주소로 내원하였다(Fig. 1A). 주변 부종이나 발적, 눈물 흘림은 없었고, 다른 안과적 이상 소견은 없었다. 안와 자기공명영상에서는 우측 안와 하내측에 경도에서 중등도로 균일하게 조영증강되는 1.0 × 0.8 × 0.7 cm 크기의 경계가 명확한 종양이 관찰되었고, 골미란이나 침범 소견은 보이지 않았다(Fig. 1B, C).

전신마취하에 결막경유접근술을 통해 안와절개술을 시행하였고, 종양이 하사근을 침범한 모습을 확인하였다(Fig. 2A, B). 절제된 조직은 1.0 × 0.6 × 0.5 cm 크기였으며, 옅은 암적색으로 부드럽게 촉지되었다(Fig. 2C). 조직검사 결과 콜라겐 기질 내에 상피세포로 구성된 거짓샘 구조(pseudglandular structure)가 관찰되었고, 두꺼운 기저막이 거짓샘 구조를 둘러싸고 있었다(Fig. 3A, B). 기질 내에 일부 사종형 석회화(psammomatous calcific foci)와 관내강의 호산성 물질도 관찰되었다(Fig. 3B). 면역조직화학 염색 결과 종양의 상피세포는 S-100에 강한 면역반응성, 사이토케라틴에 국소적인 양성 소견이었다(Fig. 3C, D). 이에 근거하여 하사근을 침범한 phkomatous choristoma로 진단하였다. 수술 후 14개월 동안 경과 관찰 중이며 재발 또는 다른 안과적 이상 소견은 보이지 않았다.

고 찰

Phakomatous choristoma의 기원은 이소성 수정체 조직으로[1,7], 병인은 명확하지 않으나 Zimmerman [1]이 이에 대한 가설을 제시하였다. 안구 발생 시기에 수정체는 표면 외배엽의 함입에 의해 형성된 수정체오목(lens pit)이 수정체소포(lens vesicle)가 되어 형성된다. Zimmerman [1]은 일부 수정체 조직이 수정체소포를 형성하는 동안 비정상적으로 파종되고, 이러한 배아성 수정체 조직이 성장하여 phakomatous choristoma 종양을 형성한다고 설명하였다. 따라서 종양은 수정체소포의 경로를 따라 어느 곳에서나 발생할 수 있다[1].

Zimmerman [1]이 처음 phakomatous choristoma에 대해 기술한 후 현재까지 27증례가 보고되었다[2]. 이 종양은 전형적으로 출생 시 발견되며 서서히 크기가 증가한다. 일반적으로 눈꺼풀의 하내측에 발생하는데, 안와 내로 침범하거나 완전히 안와 내에서 발생한 몇몇 사례도 보고되었다. 이전의 문헌에서 외안근을 침범한 경우는 Romano et al [4], Harris et al [5], 2예가 보고되었으며, 국내에서는 본 증례가 안와에 발생한 최초 보고이다.

특징적인 조직병리학적 소견은 콜라겐 기질 내의 거짓샘 구조를 둘러싸는 두꺼운 기저막, 정상 또는 백내장 수정체의 피질과 유사한 상피섬(epithelial island) 중앙의 호산성물질, 후낭하 백내장에서 보이는 “Wedl” 세포와 유사한 퇴행성 상피세포이다[1,8-10]. 상피 색소 과립과 사종형 석회화 소견도 이전 문헌에 보고되었다[9,10]. 백내장 수정체의 비정상조직 석회화의 존재는 과립 석회질 물질과 석회 소구(calcospherites) 결정체의 이차적인 축적에 의해 발생하며, 이와 동일한 기전이 phakomatous choristoma에서 비정상조직 석회화를 형성하는 데에 기여한다[6].

상피섬은 S-100과 인간 수정체의 주요한 단백질인 α, β 결정에 강한 면역반응성을 보이고[7,10,11], 비멘틴(인간 수정체상피세포에 특이적인 중간 섬유)에 양성 소견[7,10,12], 사이토케라틴(일반적으로 다른 상피세포에 존재하는 중간섬유)에 음성 소견을 보인다[10]. 이러한 소견은 phakomatous choristoma가 수정체 기원이라는 것을 뒷받침한다[7]. 사이토케라틴은 발생 초기 단계 동안 수정체에 존재하고 인간 배아 8주차 시기에 사라지는데[11], phachomatous choristoma가 S-100, 비멘틴에 양성, 사이토케라틴에 음성 소견을 보이는 것은 수정체 분화와 일치하고 성숙 수정체상피세포 소견과 동일하다[6,13]. 본 증례의 환아는 S-100에 강한 양성, 사이토케라틴에 국소적인 양성 소견을 보였는데, Blenc et al [14], Rosenbaum et al [7]도 상피세포가 사이토케라틴 AE1/AE3에 국소적인 세포질 염색을 보인 경우를 보고하였고 이것은 phakomatous choristoma의 기원이 표면 외배엽이라는 것을 반영한다[7].

Phakomatous choristoma는 보통 다른 이상을 동반하지 않으나, 눈물관 계통의 폐쇄나[12] 시신경의 형성 저하나 결손 등의[15] 선천성 안구 이상을 동반하기도 한다. 감별 질환으로 유피낭종(dermoid cyst), 누낭류(dacryocele), 영아혈관종(infantile hemangioma), 콩다래끼(chalazion), 림프 기형(lymphatic malformation), 횡문근육종(rhabdomyosarcoma)이 있다[4]. 이 중에 횡문근육종은 빠르게 성장하며 침습성이 강한 것이 특징이다. 미성숙된 악성 중간엽 세포로 이루어지며 골격근의 분화 정도에 따라 진단이 가능한데, 전사인자단백(transcription factor protein)에 해당하는 myoD1과 myogenin을 확인하여 진단할 수 있다. myoD1과 myogenin은 골격근이 발생할 때 존재하며 신경이 분포하고 근육이 성숙할 때 사라지는 인자이나, 횡문근육종에서는 이러한 인자들이 골격근에 남아 면역조직화학적 검사에서 확인이 가능하다.

Phakomatous choristoma는 매우 드물고 비특이적인 임상양상과 영상검사 결과를 보이기 때문에 보통 수술 전에 진단되지는 않으며, 수술 후 조직병리학적 검사를 통해 정확하게 진단할 수 있다. 본 증례에서도 종양의 성장 속도가 느리고 침습적인 소견이 없어 악성종양의 가능성은 낮았으나 수술 전에 정확히 진단할 수는 없었다. 절제 생검 후 시행한 면역조직화학 염색에서 데스민(desmin), myoD1에 음성이어서 횡문근육종을 배제하였고, phakomatous choristoma에 특이적인 소견을 보여 진단을 내릴 수 있었다. 치료는 완전한 외과적 절제를 통해 성공적으로 치료 가능하며[12], 불완전한 외과적 절제를 시행한 경우에도 재발이나 다른 안과적 이상 소견이 남은 경우는 보고되지 않았다[4]. 본 증례의 환아도 수술 후 14개월 동안 경과 관찰하였으나 재발이나 사시, 안구운동이상 등 다른 안과적 이상 소견은 없었다.

본 증례는 안와의 phakomatous choristoma가 하사근을 침범한 국내 최초 보고이다. 발생 빈도가 낮지만 소아의 안와 하내측에 발생한 종양 진단 시 감별진단으로 고려하여야 한다.
