Abstract

목적

최근 20년간 국내에서 발생된 진균각막염 환자의 임상양상, 원인균, 치료, 예후 등의 변화에 대해 알아보고자 하였다.

대상과 방법

본원에서 각막염으로 진단받고 시행한 각막찰과 및 배양검사에서 진균이 검출된 177안의 의무기록을 후향적으로 조사하였다. 성별, 연령, 선행 요인, 원인균, 임상양상, 치료 방법, 예후 등을 분석하였다. 또한 배양과 KOH염색 여부에 따라 KOH양성그룹, 배양양성그룹, KOH-배양동시양성그룹으로 나누어 조사하였다. 불량한 임상결과와 관련 있는 위험인자를 로지스틱 회귀분석으로 조사하였다.

결과

대상 환자의 평균연령은 67.4 ± 11.3세였고, 남자가 107명(60.5%)이었다. 진균각막염 발생의 위험인자는 안외상(57.6%), 특히 식물에 의한 외상(29.4%)이 많았다. 가장 흔한 사상진균은 푸사륨 49균주(31.6%)였고 효모균은 칸디다 17균주(11.0%)였다. 진균과 세균이 함께 배양된 41안 중 사상진균과 그람양성균이 29안(70.7%)으로 많았다. 치료 후 시력이 호전된 경우는 KOH양성그룹이 19명(79.2%), 악화된 경우는 배양양성그룹이 36명(40.4%)으로 흔하였다. 다변량분석에서 불량한 임상결과의 위험인자는 이전의 안질환(odds ratio [OR]: 3.744, p=0.010), 전방축농(OR: 2.941, p=0.030), 수술적 치료를 받은 경우(OR: 24.482, p<0.001)였다.

결론

진균각막염은 KOH만 양성이면 임상양상이 양호하였고, 배양에서 균이 확인되면 심하였다. 진균이 의심되면 초기에 KOH염색과 배양검사를 해야 하고, 불량한 임상결과의 위험인자가 있을 경우 조기에 최대한 약물치료를 시행해야 한다.

Purpose

To describe the clinical manifestations, causative organisms, treatment, and prognostic factors of fungal keratitis based on analysis of patients over the past 20 years.

Methods

A total of 177 patients (177 eyes) with fungal keratitis, who were diagnosed by smears and cultures, were reviewed retrospectively. Sex, age, predisposing factors, causative organisms, clinical manifestations, treatment, and prognosis were evaluated. The patients were divided into three groups: only potassium hydroxide (KOH)-positive, only culture-positive, and KOH- and culture-positive. Logistic regression was performed to identify the factors associated with a poor clinical outcome.

Results

The mean age of the patients was 67.4 ± 11.3 years; 107 (60.5%) patients were men. The most common predisposing factor was ocular trauma (57.6%), especially by plants (29.4%). The most prevalent filamentous fungi were Fusarium species (31.6%, 49 isolates), and the most prevalent yeasts were Candida species (11.0%, 17 isolates). In cases of bacterial and fungal keratitis, filamentous fungus with Gram-positive bacterial infection (70.7%, 29/41) was the most common presentation. Visual improvement after treatment was common in the KOH-positive group (79.2%, 19/24), while visual deterioration after treatment was common in the culture-positive group (40.4%, 36/89). Previous ocular disease (odds ratio [OR]: 3.744, p = 0.010), hypopyon (OR: 2.941, p = 0.030), and surgical treatment (OR: 24.482, p < 0.001) were risk factors for a poor clinical outcome in the multivariate logistic regression model.

Conclusions

Clinical prognosis was good in the KOH-positive only group, but poor in the culture-positive group. If fungal keratitis is suspected, diagnostic KOH staining and culture tests should be performed initially. Early intensive antifungal medical treatments are necessary for patients with risk factors for poor clinical outcomes.

진균각막염은 전 세계에서 주요한 실명의 원인 중 하나로 안구에 심각한 손상을 일으키는 질환이다[1-8]. 진균각막염의 보고는 최근 증가되고 있는데 이는 진균 동정법의 발전으로 인한 것일 수 있지만 항생제 및 스테로이드 제제의 사용 증가, 콘택트렌즈 사용의 증가, 전층각막이식과 같은 각막수술의 증가에 의한 것으로도 여겨진다[2-6]. 진균각막염은 진단이 된다 하더라도 효과적인 항진균제가 제한적이고, 항진균제의 각막 투과성이 불량하며, 각막독성이 발생될 수 있어 성공적인 치료가 어려운 상태이다[7]. 진균은 현재까지 약 5,000여 종이 분류되어 있으며 인간에게 감염을 일으키는 균은 약 100여 종으로 보고되었다[8]. 원인균에 대한 보고는 지역, 인종, 기후 등에 따라 차이를 보이며 다양한 결과들이 보고되어 있다. 인도, 네팔의 일부 지역에서는 Aspergillus species (spp.)가 가장 흔한 원인균으로 보고되었고 미국, 호주, 브라질, 터키의 일부에서는 Fuasarium spp., 일본, 영국, 미국의 일부 지역에서는 Candida spp.로 보고되었다[8-13]. 기후에 따라서는 열대와 아열대 지방에서는 실모양곰팡이(사상진균)인 FusariumAspergillus를, 온대와 한대지방에서는 효모균인 Candida를 흔한 원인균으로 보고하였다[7,8]. 국내에서는 Hahn et al [14]이 1995년부터 1999년까지 다병원연구를 통해 가장 흔한 원인균은 Fusarium spp.로 보고하였으며, Aspergillus spp., Candida spp. 순으로 많았다.

진균각막염의 진단은 전통적으로 각막찰과를 통한 염색과 배양이다. 현재 확진은 배양에 의해 주로 이뤄지고 있고 정확한 검사이다. 그러나 침습적인 조직 채취가 필요하고 배양되기까지 시간이 4주 이상 오래 걸리는 단점이 있다[15]. 반면 10% 수산화칼륨(KOH)염색은 채취를 통해 시행되지만 빠른 시간 내에 결과를 알 수 있다. 새로운 진단 방법으로 생체공초점현미경(in vivo confocal microscopy) 장비를 이용하여 진균의 균사를 확인하는 방법이 있으나 숙련된 작동자와 노련한 판독자가 있어야 하고 작동 시 환자의 협조가 필요하며 각막반흔에 의해 영향을 많이 받는 단점이 있다[16]. 이 외에도 polymerase chain reaction이나 DNA sequencing을 이용한 분자생물학적 분석도 늘어나고 있는 추세이다[7].

각막염을 치료함에 있어 초기에 정확한 병원균을 알 수 있다면 많은 도움이 된다. 외상의 병력, 콘택트렌즈 착용, 임상적 특징 소견 등이 진단에 고려되기는 하지만 진균 배양은 4주 이상의 시간이 되어야 확인되므로 검사 당일 결과를 바로 알 수 있는 KOH염색이 매우 중요하다[7]. KOH 염색 양성이거나 진균을 의심할 임상 소견이 있으면 바로 항진균치료를 시작하지만, KOH 음성인 경우 일반적으로 항생제만을 사용하게 된다. 진균각막염 환자의 임상양상을 분석할 때 이제까지 배양과 KOH염색 등 진단 방법에 따라 나누어 조사한 경우는 없었다. 저자들은 최근 20년간 본원에서 진균각막염으로 치료받은 환자들을 대상으로 진단 방법에 따라 세 그룹으로 나누어 역학조사 및 진균각막염 발생의 위험인자와 흔한 원인균, 불량한 임상결과와 관련 있는 위험인자 등을 알아보고자 하였다.

대상과 방법

2000년 1월부터 2019년 12월까지 20년간 전북대학교병원에서 진균각막염으로 진단된 후 치료를 받은 경우로 3개월 이상 추적 관찰이 가능하였던 177명 177안을 대상으로 의무기록을 후향적으로 조사하여 분석하였다. 본 연구는 헬싱키선언을 준수하며 진행되었다. 전북대학교병원 임상시험심사위원회(IRB)의 승인을 얻고 진행한 후향적 연구이다(IRB 승인 번호: 2020-07-038). 감염각막염은 기질의 침윤, 화농성 병변과 동반하여 각막상피의 결손을 보이는 것으로 정의하였다. 진균각막염은 감염각막염 중 배양검사에서 진균이 확인되거나 수산화칼륨 도말염색검사에서 양성을 보인 경우로 하였다.

진균각막염에 대하여 나이, 성별, 전신질환, 안질환, 선행 요인, 치료 방법, 경과 관찰 기간, 예후 등을 후향적 차트 연구를 통하여 조사하였다. 과거력은 각막의 외상병력, 콘택트렌즈 착용 여부, 기존의 안질환 또는 안구표면질환 유무, 굴절수술이나 각막이식술과 같은 수술 기왕력 등을 조사하였다. 각막병변의 크기는 형광염색(fluorescein staining)으로 확인된 상피결손에 대하여, 결손의 가장 긴 직선 직경과 그에 수직인 가장 큰 직경의 수치를 곱하여 도출된 직사각형의 면적으로 계산하였다[17]. 각막병변은 위치에 따라 각막중심에서 3 mm 이내에 있는 경우를 중심부(central), 각막윤부에서 3 mm 이내에 있는 경우를 주변부(peripheral)로 나누고, 그 사이에 있는 경우는 중심옆(paracentral)으로 정의하였다. 전방축농이 동반된 경우 그 높이를 각막하부 6시 방향에서 각막윤부로부터 전방축농의 상단까지 길이를 밀리미터 단위로 측정하였다.

배양검사는 감염각막염이 의심되는 경우에 이루어졌다. 일반적으로 배양 검체는 0.5% proparacaine hydrochloride (Alcaine®, Alcon laboratories, Fort Worth, TX, USA)으로 점안마취 후 개검기를 속눈썹이 닿지 않게 삽입한 후, 궤양의 기저부와 가장자리를 무균의 수술도 또는 주사바늘로 긁어내어 검체를 채취하였다. 유리 슬라이드에 도말표본을 만들어 그람염색을 시행하여 그람양성균과 그람음성균을 판정하였고 진균이 의심되는 경우에는 수산화칼륨 도말염색검사를 추가하였다. 진균에 대해서는 Sabouraud 한천배지에 검체를 접종한 후 실온에서 혹은 30°C에서 3주간 배양하였으며 배양된 진균은 슬라이드 도말검사를 통해 동정하였다. 배양검사 결과가 나오기 전이나 음성인 경우에도 배양 시 함께 시행한 KOH염색에서 양성이거나 균사가 검출되었다면 진균각막염으로 진단하고 치료하였다. 세균에 대해서는 혈액한천배지, 쵸코렛한천배지, MacConkey 한천 배지에 접종하여 37°C 온도 하에서 48시간 동안 배양을 하였다. 배양된 세균은 그람염색과 생화학적 분석에 의하여 균동정을 하였다. 진균각막염을 진단된 방법에 따라 배양검사에서만 양성인 그룹, KOH검사에서만 양성인 그룹, 배양과 KOH 모두 양성인 그룹으로 모두 세 그룹으로 나누어 임상양상을 비교 분석하였다. 또한 배양된 진균운 사상진균(filamentous fungi)과 효모균(yeast)의 세부 그룹별 임상 양상도 따로 비교하였다.

약물치료는 각막염의 중등도, 환자의 순응도 및 임상 전문의의 선호도에 따라 각 시기별로 약간씩 다르게 선택되었다. 병변이 악화되는 경우 약물치료의 실패로 판단하였고, 이러한 경우 다양한 수술적 치료가 시행되었다. 입원치료는 병변의 크기가 큰 경우, 전방축농이나 심한 각막내피덩어리(endothelial plaque)와 같은 심한 염증이 동반된 경우, 절박천공이나 수술적 치료가 필요한 경우, 안약 점안 충실도가 낮을 것으로 예상되는 경우에 시행되었다.

임상치료 결과는 최종 방문 시 교정시력뿐만 아니라 초진 때보다 시력의 감소 여부, 각막천공, 지속각막상피결손, 안내염 등의 합병증 발생 유무, 각막이식술, 안구내용제거술 등의 수술치료 시행 여부에 따라 양호, 불량의 두 군으로 구분하여 평가하였는데, 이는 Green et al [18]의 기준을 변형하여 적용하였다. 양호한 임상결과(good clinical outcome)는 각막염이 호전되어 시력이 증가하고 합병증이 없으며 수술적 치료를 받지 않은 경우로 정의하였고, 불량한 임상결과(poor clinical outcome, initial treatment failure)는 최종교정시력이 0.1 미만이거나 치료 후 시력저하가 있는 경우, 각막병변이 증가하거나 악화되어 합병증이 발생하거나 수술적 치료를 시행 받은 환자들로 정의하였다.

통계 분석은 SPSS ver. 22.0 for Windows (IBM Corp., Armonk, NY, USA)를 사용하였다. 통계 기법은 범주형 자료는 chi-square test와 Fisher’s exact test를 사용하였으며, 연속형 자료는 평균값 비교를 위해 one-way analysis of variance를 이용하였다. 통계학적 유의수준은 p값이 0.05 미만인 경우로 하였다. 불량한 임상결과의 위험인자 분석은 로지스틱 회귀분석을 이용하였고 전체군을 기준으로 분석하였다. 단변량분석에서 p값이 0.10 미만이었던 독립변수를 다변량분석에 포함하였으며, 다변량분석에서 p값이 0.05 미만인 변수를 유의한 위험인자로 간주하였다. 로지스틱 회귀분석 시 궤양의 크기는 환자 전체의 중앙값이 4.8 mm2가 나와 이를 근거로 5 mm2를 기준으로 나누었고, 초기시력은 중앙값이 logMAR 1.4로 나와 이에 해당하는 스넬렌 시력 0.04를 기준으로 나누어 분석하였다.

결 과

20년간 본원에서 진균각막염으로 진단 받은 후 시행한 각막찰과 및 배양검사에서 진균이 검출된 경우는 177명 177안이었으며, 남자가 107명(60.5%), 여자가 70명(39.5%)이었다. 직업은 농업과 관련인 경우가 90명(50.8%), 비농업과 관련된 경우는 87명(49.2%)이었다. 거주지는 도시가 58명(32.8%), 시골이 119명(67.2%)이었다. 배양양성그룹, KOH-배양동시양성그룹, KOH양성그룹 간에 성별, 직업, 거주지는 통계학적으로 차이가 없었다. 기저질환은 고혈압이 46명(26.0%), 당뇨가 26명(14.7%) 순으로 많았다(Table 1). 각막염의 발생 연령은 17세부터 95세까지로 평균 연령은 67.4 ± 11.3세였고, 60대, 70대가 가장 많았다. 30세 미만이 1명(0.5%), 30대가 2명(1.1%), 40대가 8명(4.5%), 50대가 25명(14.0%), 60대가 59명(33.3%), 70대가 61명(34.4%), 80대가 20명(11.3%), 90대가 1명(0.5%)이었다. 배양양성그룹, KOH-배양동시양성그룹, KOH양성그룹으로 나눈 연령 분포는 Fig. 1에 정리하였으며 세 그룹 모두 60대와 70대가 가장 많았다. 계절에 따른 발생양상을 분석하면 5월과 11월경에 많이 증가하였지만 월별 평균기온 변화에 따른 진균각막염 발생과의 연관성은 없었다(Fig. 2).

진균각막염의 원인균으로 총 155건의 진균이 검출되었으며 사상진균(filamentous fungi)은 132균주(85.2%), 효모균(yeast)은 21균주(13.5%), 기타 진균 2균주였다. 사상진균 중 비색소사상진균(nonpigmented filamentous fungi)은 85균주(55.5%)로 가장 많았으며, Fusarium spp., Aspergillus spp. 순이었다. 사상진균 중 색소진균(pigmented filamentous fungi)은 30균주(18.7%)였고, 이 중 Alternaria spp.가 가장 많았다. 효모균은 21균주(13.5%)였고 Candida albicans가 가장 많았다(Table 2). 진균각막염 환자에서 총 46균주의 세균이 함께 배양되었으며 그람양성균이 41균주(89.1%), 그람음성균이 5균주(10.8%)였다. 배양된 세균 중 Coagulase-negative staphylococci가 26균주(56.5%)로 가장 많았으며, 이 중에서 Staphylococcus epidermidis가 가장 많았다. Streptococcus spp.가 9균주(19.5%)로 두 번째였으며, Viridans streptococcus가 많았다(Table 3). 진균각막염 환자 중 진균과 세균이 동시에 배양된 중복감염은 41안이었으며, 사상진균 1균주와 그람양성균 1균주가 동시에 배양된 경우가 가장 많았다(Table 4).

진균 발생 양상을 5년씩 4분기로 나누어 비교하였을 때 각 분기마다 비색소사상진균이 가장 많았다. 1분기(2000-2004)에서 4분기(2015-2019)로 갈수록 발생이 증가한 경우는 색소사상진균과 효모균이었으며, 감소한 경우는 mold colony였다(Fig. 3).

진균각막염 발생의 선행요인으로는 안외상에 의한 경우가 102안(57.6%)으로 가장 많았으며, 그중에서 식물에 의한 외상, 토양에 의한 외상 순이었다. 이전의 안질환이 있던 경우는 49안(27.7%)으로 이 중 헤르페스각막염, 눈꺼풀 이상, 각막혼탁, 공막연화증, 물집각막병증 순이었다. 안과적 수술 기왕력은 15안(8.5%)이었으며, 기타 선행요인으로 점안 혹은 전신적 스테로이드 사용은 7안(4.0%), 콘택트렌즈 착용은 4안(2.3%), 원인미상은 25안(14.1%)이었다(Table 5). 진단 방법에 따른 세 그룹 간 차이를 분석하였을 때 세 그룹 모두에서 안외상으로 인한 경우가 가장 많았다. 안외상으로 인한 경우는 배양양성그룹, KOH-배양동시양성그룹, KOH양성그룹 순으로 비율이 높았는데 이는 통계적으로 의미 있는 차이를 보였다(p=0.047) (Table 5).

각막염의 위치는 전체 그룹에서는 중심부과 중심옆이 비슷하였다(Table 6). KOH양성그룹과 KOH-배양동시양성그룹에서는 중심옆이 가장 많았고, 배양양성그룹에서는 중심부가 가장 많았다. 각막궤양의 형태는 불규칙한 모양이 120안(67.8%)으로 가장 많았으며, 원형이 많았다. 다른 그룹과 달리 KOH양성그룹은 원형이 많았다. 각막침윤의 깊이는 심층부까지 침범한 경우가 110안(62.1%)이었으며 배양양성그룹은 심층부까지 침범한 경우가 75.3%인 반면 KOH양성그룹은 표층에 국한된 경우가 66.7%로 통계학적으로 의미 있는 차이를 보였다(p=0.007). 각막염의 경계가 깃털모양인 경우는 54안(30.5%)이었으며 그룹 간 유의미한 차이는 없었다. 위성 병변을 보인 경우는 전체 17안(9.6%)으로 KOH-배양동시양성그룹에서 18.8%로 높아 의미 있는 차이를 보였다(p=0.005) (Table 6).

전방축농은 70안(39.5%)에서, 각막내피반은 30안(16.9%)에서 있었고 그룹 간 차이는 없었다. 각막궤양의 평균 크기는 9.35 ± 10.57 mm2였지만 KOH양성그룹이 3.80 ± 4.43 mm2로 다른 두 그룹보다 의미 있게 작았다(p=0.017). 진균각막염 환자의 시력을 분석하였을 때 LogMAR 초기 시력은 1.42 ± 0.89였고, 치료 후 LogMAR 최종시력은 1.22 ± 1.16으로 호전되었다. 세 그룹 간 초기시력의 차이는 없었으며 최종시력은 KOH양성그룹이 0.49 ± 0.45로 가장 좋았고 이는 그룹 간 의미 있는 차이를 보였다(p<0.001) (Table 6).

진균각막염의 치료는 기본적으로 진균 안약의 사용으로 시작하였는데 amphotericin B와 fluconazole이 가장 많이 사용되었다(Table 7). 또한 동시에 사용된 항생제는 moxifloxacin이 76.8%에서 사용되었다. KOH가 양성이거나 진균의 임상적인 소견이 있는 경우에 초기 약제의 선택은 주로 fluconazole, amphotericin B를 사용하고 심한 경우 voriconazole을 사용하였으며, 동시에 세균에 대해 광범위 항생제인 fluoroquinolone를 사용하였다. 연구 초반(2000년대)에는 강화된 cefazolin과 tobramycin이 주로 배양양성그룹에서 사용되었다. 각막염이 심할 때는 추가 또는 교체로 ceftazidime과 vancomycin이 많이 사용되었다. 약물치료 외에도 전방내주사 및 결막밑주사는 50안(28.2%)에서 시행되었고, 수술치료는 총 49안(27.7%)에서 시행되었는데, 전층각막이식술, 안구내용물적출술, 각막편이식술, 양막이식술, 결막판 순이었다. 주사치료와 수술치료는 KOH양성그룹에서는 다른 그룹에 비해 낮은 빈도로 시행되었다(Table 7).

치료 전후 시력변화 양상을 분석하였을 때 진균각막염 전체 환자에서 시력이 호전된 경우(52.5%)가 시력이 악화된 경우(29.9%)보다 많았다. KOH양성그룹은 시력이 호전된 경우가 가장 많았고(79.2%), 배양양성그룹은 상대적으로 시력이 악화된 경우가 많아 세 그룹을 비교하였을 때 통계적으로 의미 있는 차이를 보였다(p=0.006) (Table 8).

배양에서 검출된 진균을 사상진균과 효모균으로 세부 분류하여 임상양상을 비교하였다(Table 9). 사상진균은 131안(86.8%), 효모균 20안(13.2%)으로 평균연령은 70세 전후였다. 사상진균은 남성(61.1%)에서 통계학적으로 의미 있게 흔히 발생하였다(p=0.032). 각막염 발생의 선행요인 분석시 사상진균은 외상인 경우가 의미 있게 많았고(p<0.001), 효모균은 이전의 안질환이 많았다. 세균 복합감염 비율이나 전방축농, 심층 침범, 수술적 치료, 시력, 초기치료 실패율 등은 두 그룹 간 큰 차이가 없었다.

진균각막염에서 불량한 임상결과의 위험인자에 대한 단변량 분석에서 통계적으로 의의가 있었던 인자들은 이전에 안질환이 있었던 경우(p<0.001), 안수술의 기왕력(p=0.014), 중심부에 발생한 궤양(p=0.008), 각막침윤이 심층까지 침범한 경우(p<0.001), 각막궤양의 크기가 5.0 mm2 이상인 경우(p<0.001), 초기시력이 0.04 미만인 경우(p<0.001), 전방축농이 있는 경우(p<0.001), 각막내피 덩어리(p=0.001), 진균각막염 치료로 수술적 치료를 받은 경우(p<0.001), 입원 치료한 경우(p=0.007)였다(Table 10). 이 인자들을 다변량 로지스틱 회귀분석을 통해 분석한 결과에서는 전방축농(odds ration [OR]: 2.941; 95% confidence interval [CI]=1.113-7.773; p=0.030), 이전의 안질환이 있었던 경우(OR: 3.744; 95% CI=1.365-10.265; p=0.010)와 수술적 치료를 받은 경우(OR: 24.482; 95% CI=4.737-126.520; p<0.001)에서 통계적으로 유의하였다(Table 10).

Risk factors for poor clinical outcome in the patients with fungal keratitis (univariate and multivariate logistic regression analysis)

고 찰

진균각막염은 수많은 종류의 사상진균과 효모균에 의하여 발생되는 전 세계적으로 흔한 질환이다[7,8]. Aspergillus spp.와 Fusarium spp.가 가장 흔한 원인진균으로 알려져 있지만 기존의 비병원성으로 알려진 Sporothrix pallida와 같은 드문 진균도 각막염을 일으킨다고 보고되었다[19]. 본 연구에서도 Plectosphaerella cucumerina, Corynespora cassiicola와 드문 진균이 검출되었다. 서로 다른 종의 진균들은 항진균제에 대하여 다양한 감수성을 보이는 것으로 알려져 있다[20]. 즉 정확한 진균을 배양하여 치료하는 것이 진균각막염 치료에서 중요한 부분이라고 볼 수 있다.

진균각막염은 특히 아시아지역에서 주요한 실명의 원인이고 열대지역에서는 감염각막염의 약 절반을 차지하기도 한다[21]. 진균각막염의 발생과 유행, 주요 원인균은 국가 간에도 차이가 있고, 한 국가에서도 지역별로도 차이가 있는데 이는 날씨, 나이, 성별, 사회경제적인 환경, 농사활동 및 도시화의 정도에 차이가 있기 때문이다. 다양한 인종과 지역들의 연구를 종합해보면 진균각막염은 남자에서 공통적으로 더 많았다. 연령은 국가와 지역별로 차이가 있었으나 보통 50세에서 60세가 가장 많았다. 열대, 아열대가 많은 인도에서는 대규모의 보고가 많은데 Rautaraya et al [22]은 인도 동부의 997명 중 남자가 70.0%로 많았고 50세에서 60세가 가장 많았다고 하였다. 또한 인도의 다른 네 지역의 연구와 비교 보고하였는데, 다른 지역들도 남자에서 호발하였고 50대가 많았다. 온대성 지역에서는 발생이 많지 않아 소규모 환자만 보고되었는데, Galarreta et al [12]은 영국 런던 67명 환자 중 남자 53.8%, 평균나이 63.7세, Farrell et al [23]은 아일랜드 지역 42명 중 남자 57.1%, 평균나이 47.4세로 보고한 반면 Nielsen et al [24]은 덴마크 지역 25명 중 남자 40.0%, 평균나이 56.7세, Watson et al [25]은 호주 시드니 51명 중 남자 65.0%, 평균나이 64세였다. 본 연구에서는 남자가 60.5%로 더 많았고 60세에서 80세까지가 가장 많아 다른 연구보다 더 고령으로 나타났는데 이는 우리나라가 전체적으로 고령화가 진행되어 있고, 진균각막염이 많이 발생하는 시골일수록 노동을 하는 고령인구가 늘어나고 젊은 인구는 감소하는 인구분포 현상이 심화된 것이 원인으로 생각된다.

배양에서 검출된 진균도 국가와 지역별로 차이가 있었으며 열대지역인 인도에서는 Aspergillus spp.가, 온대지역인 아일랜드에서는 Aspergillus spp.가, 호주에서는 Fusarium spp.가 가장 흔하였다[23,25]. 반면 영국과 덴마크 연구에서는 Candida spp.이 가장 흔하였다[12,24]. 본 연구는 Fusarium spp.가 31.6%로 가장 흔하고, Alternaria spp.가 두 번째로 많았다. 호발균은 진균각막염의 선행위험요인과도 밀접한 관계가 있었다. 안외상이 가장 많은 위험요인인 경우에는 사상진균인 Aspergillus spp.와 Fusarium spp.가 많았고, 이전의 안구표면질환이 있었던 경우에는 Candida spp.가 많았다. 또한 이전의 각막이식술이나 스테로이드 제제를 사용한 경우에도 Candida spp.가 많았다. 콘택트렌즈를 착용한 경우에는 사상진균이 많았는데 전 세계적으로 2005년부터 2006년 사이에 특정 회사의 세정제와 관련하여 Fusarium 진균각막염의 폭발적인 증가가 있었다. 특히 2000년대 초반 미국의 10개 병원 연구에서 진균각막염의 41%가 콘택트렌즈 착용자였고 Fusarium spp.가 많았다[26]. 본 연구에서는 안외상이 가장 많았고 콘택트렌즈는 매우 적었는데 이는 농사활동으로 발생한 경우가 많았기 때문이고 도시화가 상대적으로 덜 된 지역적 특성으로 생각된다. 각막염 환자에서 진균감염의 여부가 불확실할 경우에 안외상, 특히 식물성 외상의 병력이 있다면 적극적으로 항진균 치료제를 사용해야 할 것이다.

세균이 동시에 배양된 경우에 대한 보고로는 인도에서 Deshpande and Koppikar27은 10.8%였으며 이 중 Pseudomonas aeruginosa가 가장 많았고, Rautaraya et al [22]은 17%였고 Staphylococcus species가 많았다. 본 연구는 Staphylococcus species가 60.8%로 가장 많았다. 세균과 진균이 동시 배양된 경우의 조합은 사상진균과 그람양성균이 70.7%로 많아 외상으로 인한 진균감염에서 안구표면에 상주하는 그람양성균이 동시에 발현되는 경우가 흔한 것으로 생각되므로 진균각막염 시에는 그람양성균의 감염을 예방할 수 있는 항생제를 함께 사용하는 것이 좋겠다.

진균각막염의 진단은 최종적으로 배양검사를 통해 정확하게 이뤄지고 있으나 배지 위에서 성장하여 확진이 나오기까지 약 4주 이상이 걸려서 각막염 초기에 정확한 진단정보를 얻을 수 없다. 각막찰과를 통해 얻어진 검체를 빠른 시간 내에 진단하는 대체 방법으로 10% 수산화칼륨 염색이 시행되고 있는데 Bharathi et al [28]은 3,298안에서 99.3% 민감도, 99.1% 특이도, 98.5% 양성예측도, 99.6% 음성예측도를 보여 높은 진단적 가치가 있는 빠른 검사라고 하였다. 이에 반해 Zhang et al, [29] Badiee et al, [30] Vengayil et al [31]은 각각 81%, 68%, 60%의 상대적으로 낮은 민감도를 보인다고 하였는데, 이는 불충분한 검체 채취, 궤양 크기가 작은 경우, 염색 후 진단하는 사람의 경험 미숙 등으로 유발될 수 있다고 하였다[8]. 세균은 배양 후 이틀부터 결과를 확인할 수 있지만, 진균은 검체 채취 후 대개 한 달 후에 확인이 되어 각막염치료에 도움이 되지 못하고 늦게 진단이 되는 경우가 많다. 반면 KOH염색은 바로 당일 날 결과를 확인하므로 항진균치료를 바로 시작할 수 있다. 따라서 본 연구에서는 진단 방법에 따른 임상양상의 차이를 확인해보기 위해 KOH양성그룹, 배양양성그룹, KOH-배양동시양성그룹의 세 그룹으로 조사하였는데 KOH양성그룹이 안외상에 의해 발생이 많고 안구표면질환으로 인한 경우는 드물었다. 또한 궤양의 크기도 작고 표층만 침범한 경우가 많고 중심 옆에 호발하고 수술적 치료를 받은 경우는 드물어 최종시력이 배양양성그룹, KOH-배양동시양성그룹보다 좋았다. 최종시력은 KOH양성그룹은 시력이 호전된 경우가 가장 많았고, 배양양성그룹은 상대적으로 시력이 악화된 경우가 많았으며 KOH-배양동시양성그룹은 시력이 호전된 경우가 더 많았다. 배양양성그룹의 경우 세균중복감염이 많았고, 이 경우에는 초기부터 세균각막염으로 치료받고 항진균제를 사용하지 않은 경우가 많은 것도 원인으로 생각된다. 이를 종합하여 보았을 때 KOH만 양성인 경우는 치료 초기부터 진균을 의심하여 조기에 항진균 점안치료를 시작하였고, 실제 진균이 배양이 되지 않을 만큼 침투한 진균의 수가 적은 상태였을 가능성이 높아 상대적으로 치료 효과가 좋았을 것으로 생각된다. 그러므로 초기에 각막찰과검사로 KOH 양성이면 적극적인 항진균제치료를 시행하는 것이 환자의 예후에 좋다. 또한 KOH염색 양성의 민감도를 높이기 위해 피부과의 확인을 받는 것이 진균각막염의 초기 치료 결정에 도움이 될 것으로 생각된다.

사상진균과 효모균에 의해 발생한 진균각막염을 비교하였을 때 Galarreta et al [12]은 선행위험요인으로 효모균은 안구표면질환, 전층각막이식, 스테로이드제제의 사용으로 인한 경우가 많았고 사상진균은 안외상, 콘택트렌즈 착용이 많아 통계학적으로 의미 있는 차이를 보인다고 하였으며, 나이나 성별, 발생계절, 치료 등은 차이가 없었다. Watson et al [25]도 효모균은 안구표면질환에서, 사상진균은 안외상, 콘택트렌즈 착용이 많다고 하였으나, 스테로이드제제 사용, 전층각막이식은 차이가 없다고 하였다. 본 연구에서도 사상진균의 경우 안외상으로 인한 경우가 의미 있게 많았으나 다른 임상양상은 차이가 없었다. 또한 다른 연구와 달리 사상진균이 남자에서 더 많이 발생하여 의미 있는 차이를 보였다.

진균각막염의 치료실패의 위험인자에 대한 보고들도 있다. Lalitha et al [32]은 115명에서 다변량 분석 결과 각막궤양 크기가 14 mm2로 크거나, 전방축농, Aspergillus가 동정된 경우에 위험성이 높다 하였고, Lee et al [33]은 109명에서 궤양 크기가 10 mm2 보다 큰 경우와 이전의 안질환을 위험인자로 보고하였다. Sharma et al [34]은 환자의 연령, 치료 약제의 종류, 각막 상피결손의 크기가 최종시력을 결정하는 필수요소라고 하였고 Rautaraya et al [22]은 증상 발생 10일 이전에 내원한 경우보다 증상 발생 10일 이후에 늦게 내원한 환자에서 전층각막이식술과 같은 수술치료가 통계학적으로 유의미하게 많았다. 본 연구에서는 불량한 임상결과의 위험인자로 전방축농, 이전의 안질환이 있었던 경우와 수술치료를 받은 경우에서 통계적으로 유의하였다. 여러 연구들을 종합해 보았을 때 지역별로 다를 수 있으나 진균각막염은 연령, 증상 발생 후 내원한 시기, 전안부의 임상양상, 이전의 안질환, 진균각막염으로 인해 수술치료를 받은 경우 등을 나쁜 예후의 위험인자로 생각할 수 있다.

본 연구는 177안의 비교적 많지 않은 환자를 대상으로의무기록을 후향적으로 조사했다는 점, 다병원연구가 아니라 한 지역의 3차 병원 환자를 대상으로 하였다는 점, 병원 사정상 항진균제 감수성 검사가 시행되지 않았다는 한계가 있다. 그러나 배양과 KOH염색검사에서 진균감염으로 확진된 환자군만을 대상으로 하여 진균각막염의 치료에 있어 불량한 예후를 나타낼 수 있는 위험인자들을 분석하였다. 따라서 본 연구를 적극적 치료의 시행 여부 결정 및 치료 예후를 예측할 수 있는 근거로 사용할 수 있다고 생각된다.

결론적으로 20년간의 후향적 조사에서 진균각막염은 KOH만 양성이면 임상양상이 양호하였고, 배양에서 균이 확인되는 경우에는 임상양상이 비교적 심하였다. 진균이 의심되면 초기에 KOH염색과 배양검사를 해야 하고, 전방 축농과 이전의 안질환과 같은 불량한 임상결과의 위험인자가 있을 경우에는 조기에 최대한 약물치료를 시행해야 할 것이다.
