2017년 6차 International Workshop for Ocular Sarcoidosis (IWOS)에서 개정 발표된 눈 사르코이도시스 진단 기준을 2009년 발표된 1차 IWOS 진단 기준과 비교 분석하고, 이를 한국인 눈 사르코이도시스 진단에 활용했을 때의 임상적 효용성을 알아보고자 한다.
2007년부터 2018년까지 서울아산병원 안과에서 포도막염으로 진단받은 환자들 중 호흡기내과 및 안과 정밀검진을 시행 후 눈 사르코이도시스로 진단된 환자들의 의무기록을 후향적으로 분석하였다. 내과에서 시행한 혈액검사, 영상학적 검사 및 조직검사 결과와 안과 검진 시 시행한 세극등현미경검사, 안저검사, 형광안저 혈관조영술 결과를 종합하여 2009년 및 2017년 IWOS 진단 기준에 따라 눈 사르코이도시스를 진단하였다.
눈 사르코이도시스로 진단된 총 34명의 의무기록을 분석하고 두 진단 기준을 활용하여 분류하였다. 조직검사를 시행한 32명의 환자 중 31명이 조직검사상 사르코이도시스로 진단되었다(양성률 96.87%). 31명이 definite ocular sarcoidosis (OS), 2명이 presumed OS로 이전 진단 기준과 개정된 진단 기준에서 동일하게 분류되었고, 1명의 환자만 기존 진단 기준에서는 possible OS, 개정된 진단 기준에서는 presumed OS로 분류되었다.
조직검사 양성률이 높고, 영상학적 검사의 질이 높은 우리나라의 임상상황에서는 이전 진단 기준과 비교하여 개정된 진단 기준을 활용하였을 때 진단 결과에 큰 차이는 없을 것으로 사료된다.
To compare differences between the original criteria for diagnosis of ocular sarcoidosis (OS) (first International Workshop for OS [IWOS] 2009) and the revised criteria (sixth IWOS 2017), and their clinical usefulness when assessing OS in Korean patients.
We analyzed patients with suspected OS who visited our tertiary referral ophthalmological and pulmonary clinic from 2007 to 2018. We diagnosed patients using both sets of criteria. Blood test and biopsy data (collected by physicians) and slit-lamp, fundus, and fluorescein angiography data (collected by ophthalmologists) were reviewed.
Thirty-four patients were diagnosed using both criteria. Of 32 patients who underwent biopsies, 31 had OS (96.87%). Using either set of criteria, 31 patients were diagnosed with definite OS and two with presumed OS. One patient diagnosed with possible OS using the previous criteria was diagnosed with presumed OS using the revised criteria. The new criteria add the lysozyme level, the CD4/CD8 ratio, and positron emission tomography imaging data to the old criteria and add the descriptors “presumed OS” and “probable OS”. There is no need to use the revised criteria in Korea; the biopsy and imaging data are adequately diagnostic.
IWOS revised the OS diagnostic criteria by adding new parameters. However, this was unnecessary for Korea, where the biopsy and imaging data are adequately diagnostic.
사르코이도시스는 원인 미상의 다발성 염증성 질환으로, 주로 폐, 림프절, 피부 및 눈에 영향을 미친다[
하지만 IWOS에서 제시한 기존 진단 기준이 여러 연구들을 통해 한계점들이 지적되었다. 일본에서 시행된 기존 진단 기준의 타당성에 관한 연구를 보면 전신적 검사에 해당하는 간기능검사 이상 소견의 경우 낮은 빈도를 보였으며 이를 제외한 나머지 검사 지표들은 조직학적으로 진단된 사르코이도시스 환자에서 높은 빈도를 보였다[
이러한 한계점을 극복하기 위해 IWOS에서는 리소자임(lysozyme), CD4/CD8 ratio, 양전자 방사단층촬영법(positron emission tomography) 등의 다양한 전신적 검사들을 추가하고 진단 분류를 개정하여 2017년에 새로운 눈 사르코이도시스 진단 기준을 제안하였다[
본 연구는 2007년 1월부터 2018년 12월까지 서울아산병원 안과에서 내인성 포도막염을 진단 받은 환자 중 안과 및 호흡기 내과에서 진단 검사를 시행한 후 눈 사르코이도시스로 진단된 환자들의 의무기록을 후향적으로 분석하였으며, 본원의 연구윤리심의위원회(Institutional Review Board, IRB) 승인하(IRB 승인 번호: 2019-0560) 수행하였다. 안과 방문 시 세극등현미경검사, 안저검사, 형관안저 혈관조영술을 시행하였으며, 이를 기반으로 다음과 같은 안과적 임상양상이 있는지 살펴보았다: 각막후면침착물, 홍채결절 및 텐트 모양의 주변홍채앞유착, 눈덩이 혹은 진주사슬모양 혹은 유리체혼탁, 주변부의 맥락망막증상, 정맥주위염 혹은 거대동맥류, 시신경결절 혹은 육아종, 병변의 양측성 여부. 전신 검사에서는 흉부 X선 촬영, 흉부 컴퓨터단층촬영 등의 영상학적 검사, 혈액검사 및 조직검사 결과를 기존 진단 기준과 개정 진단 기준 각각에 적용하여 눈 사르코이도시스 진단 가능성을 알아보았다. 환자들은 기존 진단 기준에서 제시한 네 가지 진단 분류 및 개정 진단 기준의 세 가지 진단 분류로 나뉘어졌으며(
총 34명의 환자들이 기존 및 개정 눈 사르코이도시스 진단 기준에 따라 분류되었다. 눈 사르코이도시스 환자의 인구학적 분포는 다음과 같다(
기존 진단 기준에 따라 전신적 검사 결과를 살펴 보면(
개정 진단 기준에서는 기존 진단 기준과 비교하여 양측 폐문 림프절병증에 대한 영상학적 검사로서 흉부 X선 촬영과 흉부 컴퓨터단층촬영으로 나뉘어졌던 항목이 합쳐졌으며, 이에 대하여 모든 환자의 영상학적 검사에서 양측 폐문림프절병증 양성으로 분류되었다(100%). 새롭게 추가된 진단 기준들 중 폐의 조직적 변화(78.8%)가 높은 양성률을 보였으며, 양전자 방사단층촬영은 4명의 환자에서 진행되어 모두 이상 소견을 보였다(100%). 새롭게 추가된 진단의학검사 중 혈청 리소자임 및 기관지 세척액에서의 CD4/CD8 ratio 분석이 시행된 환자는 없었다.
34명의 환자들 중 2명을 제외하고 조직검사가 시행되었으며(

본 연구에서는 한국 3차 의료기관에서 눈 사르코이도시스로 진단 받은 환자들의 임상양상을 확인하고, 2009년에 만들어진 눈 사르코이도시스 진단 기준과 2017년에 개정된 진단 기준을 눈 사르코이도시스를 진단 받은 환자들에게 적용하여 새로운 진단 기준의 효용성을 비교 분석하였다. 우선 인구학적인 분포를 보았을 때, 이전 연구에서 전체 사르코이도시스 중 눈 사르코이도시스가 차지하는 비율은 약 20%였다[
안과적 임상양상을 살펴보면 본 연구에서는 포도막염이 양측성으로 발현한 경우가 94.1%로 높게 나타났다. 이는 다른 연구에서도 비슷한 경향을 보였으며 비록 양측성이 비특이적인 특성이지만 단안에 호발하는 특발성 포도막염에 비해 눈 사르코이도시스에 있어서 중요한 요소임을 알 수 있다[
개정된 전신검사 기준에서는 우선 양측 폐문 림프절병증을 판단에 흉부 X선 촬영과 흉부 컴퓨터단층촬영으로 나뉘어졌던 기준이 하나로 합쳐졌고, 폐의 조직적 변화가 추가되었다. 이에 양측 폐문 림프절병증의 경우 양성률이 1.00, 폐의 조직적 변화가 0.79로 높은 양성률을 보였는데, 이는 폐와 흉부내 림프절이 사르코이도시스에서 가장 많이 나타나는 기관임을 반영하는 결과라 할 수 있다[
개정 분류 기준에서 새롭게 추가된 전신 검사들 중 폐의 조직적 변화 외에 대부분 시행률이 매우 낮아 이에 대한 유용성을 판단하기 어려웠다. 혈청 리소자임과 기관지 세척액에서의 CD4/CD8 ratio는 모든 환자에서 시행되지 않았는데 혈청 리소자임의 경우 본 기관에서 시행되지 않고 있으며, 기관지 세척액에서의 CD4/CD8 ratio는 기관지 세척액에서 림프구 비율이 15% 미만으로 그 분석을 시행하지 않았다. 또 다른 추가된 진단 기준인 양전자 방사단층촬영의 경우 오직 네 명의 환자에서만 시행되었다. 낮은 시행률로 인해 검사의 유용성을 판단하기 어렵지만, 양전자 방사단층촬영의 이상 소견이 높은 양성률을 보였고 전신적 사르코이도시스에서 양전자 방사단층촬영의 이상 소견이 유용하게 이용되는 만큼 이에 대한 추가적인 연구가 필요할 것으로 보인다[
조직검사의 경우 대부분의 환자에서 시행되었고 주로 폐 조직과 폐문부 림프절에서 양성이 나왔다(n=27, 87%). 이는 사르코이도시스에서 폐 사르코이도시스가 가장 많이 나타나기 때문에 조직검사가 많이 시행되었고, 또한 기관지 EBUS를 이용하여 채취하는 방법이 도입되면서 더욱 더 조직검사의 양성률을 높일 수 있을 것으로 사료된다[
위의 내용을 종합해보면 개정 진단 기준에서는 새로운 전신 검사들이 많이 추가되었지만 본 연구의 눈 사르코이도시스 진단에는 큰 영향을 미치지 못했다. 그 이유는 눈사르코이도시스 진단에 있어서 조직학적 근거와 양측 폐문림프절병증이 가장 중요하게 작용하는데, 이번 연구에서는 대부분의 환자들이 조직검사를 시행하였으며 양성률이 매우 높았기 때문이다. 영상적 평가인 양측 폐문 림프절병증의 경우 흉부 X선 촬영으로만 판단하였을 때에는 양성률 0.79였으나 흉부 컴퓨터단층촬영까지 포함하였을 때 모든 환자에서 나타나 컴퓨터단층촬영의 진단적 유용성을 확인 할 수 있었다. 조직검사를 시행하기 어렵고 영상학적 진단이 어려운 경우에 probable OS로 진단하여 눈 사르코이도시스의 감별 진단에 도움을 주고자 하였으나, 조직검사 시행률 및 양성률이 높은 한국의 진료 환경에서는 눈 사르코이도시스를 진단하는데 임상적으로 기존 진단 기준과 개정 진단 기준과의 거의 차이를 보이지 않았다.
이번 연구의 한계점으로는 후향적 연구 방식으로 인해 개정 분류 기준에서 새롭게 추가된 검사들을 환자군에 모두 시행할 수 없었다는 점, 그리고 단일 기관의 환자군에 국한되어 다양한 진단법을 적용할 수 없었다는 점을 볼 수 있다. 특히 눈 사르코이도시스 진단에 있어서 조직검사와 양측 폐문 림프절병증을 주로 활용하여 진단하였기에, 조직검사와 영상검사가 없는 상태에서, 전신적 검사의 양성소견이 크게 영향을 미치는 probable OS에 대해서는 판단하기 어려운 점이 있다. 이에 대해서는 추후 전향적 방식의 추가적인 분석이 필요할 것으로 보인다.
기존 분류 기준과 개정 분류 기준으로 나누어 총 34명을 진단 분류하였을 때 possible OS에서 presumed OS로 재분류된 1명 외에 진단이 달라진 점은 없었다. 본 연구에서 눈사르코이도시스의 진단에 있어 조직검사 결과와 영상학적 검사 여부가 가장 중요하게 작용하였다. 이에 조직검사 양성률이 높고, 영상학적 검사의 질이 높은 한국의 임상 상황에서는 기존 진단 기준과 비교하여 개정된 진단 기준을 활용하였을 때 진단 결과에 있어 큰 차이는 없었다.
