한국인에서 리바운드안압계의 임상적 유용성

목적

리바운드안압계 Icare ic200으로 측정한 안압을 골드만압평안압계로 측정한 안압과 비교하여 임상에서 Icare ic200의 유용성을 확인하고자 하였다.

대상과 방법

전체 한국인 294명, 294안이 연구에 포함되었다. 모든 환자에서 Icare ic200으로 안압을 측정한 뒤 골드만압평안압계로 안압을 측정하였다. 두 안압계의 안압 일치도를 평가하였으며, 골드만압평안압계의 22 mmHg 이상에 대한 Icare ic200의 진단능을 분석하였다. 생체계측을 포함한 임상인자들이 두 안압계로 측정된 안압의 차이에 영향을 미치는지 분석하였다.

결과

Icare ic200의 안압과 골드만압평안압계의 안압은 강한 양의 상관관계를 보였다(r=0.875, p<0.001). Icare ic200의 안압이 골드만압평안압계의 안압보다 평균 3.07 ± 2.67 mmHg 낮았고, Bland-Altman plots에서 95.24%의 환자가 95% 일치한계(-2.16 to 8.30 mmHg) 안에 분포했다. 22 mmHg 이상의 고안압에 대한 Icare ic200의 진단능은 0.959 (area under the receiver operating characterisitic)였다. 다변량회귀분석 결과 나이가 많은 것(β=0.034, p=0.020)과 각막곡률이 높은 것(β=0.213, p=0.030)이 두 안압계의 차이가 큰 것과 연관된 것으로 나타났다.

결론

Icare ic200은 골드만압평안압계와 좋은 일치도 및 고안압에 대한 높은 진단능을 보였으나, 평균 3 mmHg 낮게 측정되었다. 따라서 Icare ic200은 임상에서 골드만압평안압계를 대체해서 사용하기보다는 고안압에 대한 선별검사에 유용하게 사용될 수 있을 것으로 보인다.

ABSTRACT

Purpose

To evaluate the efficacy of the Icare ic200 in clinical practice by comparing the intraocular pressure (IOP) measured with the Icare ic200 rebound tonometer to the IOP measured with the Goldmann applanation tonometer (GAT).

Methods

A total of 294 eyes of 294 Korean patients were included. IOP was measured with the Icare ic200 and then measured again with a GAT in all patients. We evaluated the degree of IOP agreement between the two tonometers and analyzed the diagnostic ability of the Icare ic200 for a reading ≥ 22 mmHg with the GAT. We also analyzed whether clinical factors including biometry affected the difference in IOP measured by the two tonometers.

Results

The IOP values measured with the Icare ic200 and GAT were strongly correlated (r = 0.875, p < 0.001). The IOP measured with the Icare ic200 was lower than the IOP measured with GAT. The mean difference was 3.07 ± 2.67 mmHg, and 95.24% of patients were distributed within the 95% limits of agreement (-2.16 to 8.30 mmHg) on Bland-Altman plots. The diagnostic ability of the Icare ic200 for IOP ≥ 22 mmHg was 0.959 (area under the receiver operating characterisitic). In multivariate regression analyses, older age (β = 0.034, p = 0.020) and greater corneal curvature (β = 0.213, p = 0.030) were correlated with larger IOP differences between the two tonometers.

Conclusions

Although the Icare ic200 was more consistent than the GAT with reasonable diagnostic ability for ≥ 22 mmHg, the IOP measured 3 mmHg lower than the GAT. Therefore, the Icare ic200 might be more useful as a screening test to increase IOP rather than replacing GAT in clinical practice.

녹내장은 진행성 시신경병증이며, 전 세계적으로 실명의 주요한 원인 질환 중 하나이다[1]. 녹내장의 치료 표적은 아직까지는 안압이 유일하며, 녹내장 환자에서 시행되는 다양한 약물 치료, 레이저 치료, 수술 치료의 목적은 안압을 낮추는 것이다. 따라서 치료 전과 후의 정확한 안압 측정은 치료 효과를 판단하는데 매우 중요하다[2].
골드만압평안압계는 안압을 측정하는 표준 검사계로 인정받고 있으며, 안압을 낮추는 것이 녹내장의 진행을 늦추는 데 효과가 있음을 보고한 대규모 연구들에서도 골드만압평안압계를 사용하였다[3-6]. 골드만압평안압계는 임상에서도 널리 사용하고 있는데, 몇 가지 제한점을 가지고 있다. 첫째로 세극등현미경검사계에 부착되어 있기 때문에 똑바로 앉은 상태에서만 검사가 가능하여 거동이 불편한 환자나 고령이나 어린이에서는 측정이 어렵다. 또한, 안압 측정시 이중프리즘이 각막의 넓은 면적에(7.35 mm2) 접촉해야 하기 때문에 점안마취가 필요하고, 전염성 질환의 전파 가능성을 가지고 있다[7].
최근에 소개된 리바운드안압계는 골드만압평안압계에 비해 안압검사시 각막에 접촉되는 탐침의 면적이 매우 좁고 접촉 시간이 짧기 때문에 점안마취가 필요하지 않다는 장점을 가지고 있다[8-11]. 또한, 초기 모델은 안압계의 장축을 지면에 대해 수직 또는 수평으로 위치시켜야만 안압 측정이 가능하다는 단점을 가지고 있었으나, 이를 보완한 새로운 모델이 출시되어 임상에서 사용하기에 보다 편리해졌다. 본 연구에서 사용한 Icare ic200® (Icare Finland Oy, Helsinki, Finland)은 200°의 범위 내에서는 환자가 비스듬하게 눕거나 앉은 자세에서도 안압 측정이 가능하다[12,13]. 최근 Badakere et al [14]은 156명, 156안을 대상으로 Icare ic200과 골드만압평안압계로 측정한 안압이 높은 일치도를 보였다고 하였는데, 아직까지 한국인에서 Icare ic200과 골드만압평안압계를 비교한 연구는 보고된 바 없다. 본 연구에서는 다양한 생체계측을 가진 한국인을 대상으로 Icare ic200과 골드만압평안압계의 일치도를 확인하고자 하였으며, Icare ic200의 고안압에 대한 진단능을 분석함으로써 임상적 유용성을 평가하고자 하였다.

대상과 방법

2020년 2월 4일부터 2020년 2월 25일까지 녹내장 클리닉을 방문한 만 19세 이상의 환자를 연구 대상으로 하였다. 본 연구는 헬싱키선언을 준수하였으며 임상시험심사위원회(Institutional Review Board, IRB)의 승인을 받았고(승인번호: 2020-07-110), 후향적으로 진행되어 환자 동의서는 면제되었다.
본원에서는 안과를 방문한 모든 성인 환자에서 진료에 앞서 시력검사와 비접촉식안압검사를 시행하고 있는데, 녹내장 클리닉을 방문한 환자들에서는 비접촉식안압계 대신 Icare ic200을 사용하였다. 모든 Icare ic200 검사는 한 명의 숙련된 검사자가 시행하였으며, 환자는 점안마취 없이 앉은 자세로 정면을 주시하도록 한 후 안압을 측정하였다. 한 번 검사시 연속적으로 측정된 안압들의 표준편차 범위가 95% 이내로 표시되는 경우의 평균 안압값을 기록하였으며, 두 번 검사하여 평균 값을 분석에 사용하였다. Icare ic200 검사와 골드만압평안압검사는 각각 한 명씩의 숙련된 검사자가 시행하였으며, 두 안압검사 사이의 경과 시간은 최소 10분 이상이었다. 환자에게 Alcaine® (Proparacaine HCL 0.5%; Alcon, Inc., Fort Worth, TX, USA)과 Fluorescite® (Fluorescein 10%; Alcon, Inc., Fort Worth, TX, USA)를 점안한 후 골드만압평안압계로 안압을 측정하였고, 두 번 측정한 안압의 차이가 2 mmHg 이내인 경우 평균값을 기록하였고, 2 mmHg 이상 차이가 나는 경우 다시 안압을 측정하여 기록하였다.
각막부종이나 혼탁, 이상증, 변성증이 있는 환자와 최근 3개월 이내에 안과적 수술을 받은 기왕력이 있는 환자는 연구에서 제외하였다. 안압검사에 대한 협조도가 낮은 환자도 연구에서 제외하였다. 양안 중 기준을 만족하는 눈을 대상으로 하였으며, 양안이 기준을 만족하는 경우에는 Microsoft Excel® (Microsoft Co.,Ltd., Redmond, WA, USA)을 이용한 무작위표본추출 방법으로 선택된 눈을 대상으로 하였다. 전자의무기록에서 대상 환자의 중심각막두께(Pocket II®; Quantel Medical, Cournon-d’Auvergne, France), 안축장과 각막곡률(IOL Master 500®; Carl Zeiss Meditec AG, Jena, Germany)을 포함한 임상인자들을 후향적으로 조사하였다.
통계학적 분석은 SPSS version 19.0 (IBM Corp., Armonk, NewYork, USA)를 이용하였다. Bland-Altman plots을 이용하여 두 가지 안압계로 측정한 안압의 일치도를 평가하였으며, 골드만압평안압계로 측정한 안압이 22 mmHg 이상인 경우에 대한 Icare ic200의 진단능을 area under the receiver operating characterisitic (AUROC) curves를 이용하여 분석하였다. 선형회귀분석을 이용하여 환자의 생체계측치를 포함한 임상인자들이 두 가지 안압계로 측정한 안압의 차이에 영향을 미치는지를 확인하였으며, 유의수준이 0.05 미만인 경우를 통계적으로 유의한 것으로 판단하였다.

결 과

전체 294명, 294안이 연구에 포함되었으며, 평균 나이는 64.54 ± 12.72세였고, 남자가 168명, 여자가 126명이었다. 정상인과 녹내장 환자가 각각 86명과 208명이었다. 골드만압평안압계로 측정한 안압과 Icare ic200로 측정한 안압의 평균값은 각각 16.73 ± 5.05 mmHg (범위 5-46 mmHg)와 13.67 ± 5.49 mmHg (4-46.4 mmHg)였다. 중심각막두께는 538.88 ± 39.69 μm였으며 안축장은 24.17 ± 1.87 mm였다(Table 1).
Fig. 1는 Bland-Altman plots으로 두 안압계로 측정한 안압의 평균과 안압의 차이 사이에는 유의한 상관관계를 보였으며, 두 안압계의 평균 안압이 높을수록 안압의 차이가 줄어드는 것으로 나타났다(r=-0.169, p=0.004). 294명 중 280명(95.24%)이 95% 일치한계(limits of agreement, -2.16 to 8.30 mmHg) 안에 분포하여 좋은 일치도를 보였다. Icare ic200의 안압이 골드만압평안압계의 안압에 비해 평균 3.07 ± 2.67 mmHg 낮게 측정되었다.
두 안압계로 측정한 안압 사이에는 강한 양의 상관관계를 보였다(r=0.875, p<0.001). 골드만압평안압계의 안압이 22 mmHg 미만인 군(n=261)과 22 mmHg 이상인 군(n=33)으로 나누었을 때 두 안압계로 측정한 안압의 상관계수는 각각 r=0.671 (p<0.01)과 r=0.931 (p<0.01)이었으며, 22 mmHg이상인 군에서 두 안압계의 상관계수가 유의하게 더 큰 것으로 나타났다(Fisher’s Z transformation, Z=4.43, p<0.001, Fig. 2).
두 안압계의 안압 차의 평균값은 골드만압평안압계의 안압이 22 mmHg 미만인 군과 22 mmHg 이상인 군에서 각각 3.13 mmHg (95% limits of agreement, -2.06 to 8.32 mmHg)와 2.54 mmHg (-3.06 to 8.14 mmHg)였으며, 두 군 사이에 유의한 차이는 없는 것으로 나타났다(p=0.232, Fig. 3).
골드만압평안압계로 측정한 22 mmHg 이상의 고안압에 대한 Icare ic200의 진단능(AUROC)은 0.959 (95% 신뢰구간, 0.93-0.99)였다(Fig. 4A). 민감도(90.9%)와 특이도(83.9%)가 가장 높은 절사점(cut off value)은 15.65 mmHg였다. 민감도가 84.8%이고 특이도가 94.6%일 때는 17.80 mmHg였으며, 민감도가 81.8%이고 특이도가 95.0%일 때는 18.05 mmHg였다. Icare ic200의 안압 17.80 mmHg와 18.05 mmHg를 기준으로 하였을 때 각각 5명(15%)과 6명(18%)에서 골드만압평안압계의 안압이 22 mmHg보다 높은 경우를 놓쳤다(Fig. 4B).
단변량선형회귀분석에서 Icare ic200과 골드만압평안압계로 측정한 안압의 차이는 나이(β=0.049, p<0.001), 안축장(β=-0.185, p=0.034), 각막곡률(β=0.292, p<0.001)과 유의한 연관성을 가지고 있는 것으로 나타났다(Table 2, Fig. 5). 이들 세 가지 임상인자와 골드만압평안압계의 안압에 영향을 주는 것으로 알려져 있는 중심각막두께를 포함시킨 다변량선형회귀분석에서는 나이가 많은 것(β=0.034, p=0.020)과 각막곡률이 높은 것(β=0.213, p=0.030)만이 두 안압계의 안압 차이가 커지는 것과 유의하게 연관된 것으로 나타났다(Table 2).

고 찰

본 연구의 결과 Icare ic200과 골드만압평안압계로 측정한 안압은 일치도가 높았으나 Icare ic200의 안압이 골드만압평안압계에 비해 평균 3 mmHg 정도 낮게 측정되었다. 골드만압평안압계의 22 mmHg 이상의 고안압에 대한 Icare ic200의 진단능은 높았으며(AUROC, 0.959), 절사점을 18 mmHg로 하였을 때 골드만압평안압계의 22 mmHg 이상인 환자를 놓치는 경우는 18%였다.
임상에서 안압을 측정하는 것은 다음의 몇 가지 측면에서 중요성을 가진다. 먼저, 시신경에 이상이 없는 사람에서도 고안압은 향후 녹내장으로 발전할 수 있는 대표적인 위험요인 중 하나이기 때문에 안압이 정상보다 높은 경우 정기적인 안과 검진이 요구된다. 또한, 녹내장성 시신경병증이 있는 환자에서는 치료 전 안압을 아는 것이 예후를 예측하는 데 도움이 되며, 치료 효과를 판단하기 위해 정기적인 안압검사는 필수적이다[11]. 전자에서는 사용이 편리하고 고안압을 잘 진단해낼 수 있는 안압계가 유용하게 사용될 수 있으며, 후자에서는 안압을 정확하게 측정할 수 있는 안압계가 유용하게 사용될 수 있다. 골드만압평안압계는 현재까지 가장 정확히 안압을 측정할 수 있는 표준검사로 인정 받고 있으며, 임상에서도 널리 사용되고 있으나 똑바로 앉은 자세를 유지하기 어려운 환자에서 검사가 어렵고, 각막에 질환이 있는 환자에서 검사가 제한적이다. 이 때문에 사용이 편리하면서도 안압을 정확히 측정할 수 있는 새로운 안압계에 대한 요구가 높은 실정이다.
2000년에 소개된 리바운드안압계는 일회용 탐침의 각막에 대한 반동력을 안압으로 환산한 결과와 검사의 신뢰도 수준을 같이 보여주는데, 검사 시간이 짧고 휴대가 가능하며 점안마취 없이도 검사가 가능하여 임상에서의 사용이 증가하고 있다[9,15-23]. Kageyama et al [9]은 생후 6개월에서 15세의 건강한 소아를 대상으로 선별검사에 널리 사용되고 있는 비접촉식안압계인 NT-2000® (Nidek, Gamagori, Japan)와 Icare PRO를 비교하였는데, 안압 측정이 가능했던 환자의 비율이 NT-2000 (72.2%)을 사용한 경우보다 IcarePRO (88.9%)를 사용한 경우에서 높아 협조도가 떨어지는 환자의 안압 선별검사도구로 리바운드안압계가 고려될 수 있을 것이라고 제안하였다. 이전 모델인 Icare TA01i® (Tiolat Oy, Helsinki, Finland)와 IcarePRO® (Icare Finland Oy, Helsinki, Finland)에 비해 본 연구에서 사용한 Icare ic200은 200° 이상의 넓은 측정 범위를 지원하여 거동이 불편한 환자에서 안압을 측정하는데 보다 유용하게 사용할 수 있다[12,14].
리바운드안압계와 골드만압평안압계를 비교한 이전 연구 결과들을 보면 Munkwitz et al [17]은 Icare TA01i의 안압이 골드만압평안압계보다 평균 0.79 mmHg (95% 일치한계, -8.67 to 10.25 mmHg) 높다고 하였다. 이 연구에서 안압이 23 mmHg 이상(23-60 mmHg)인 군과 22 mmHg 이하(16-22 mmHg)인 군으로 나누었을 때 전자에서는 Icare TA01i가 0.04 mmHg (95% 일치한계, -7.34 to 7.42 mmHg) 높았는데 후자에서는 0.28 mmHg 낮았고, 95% 일치한계도 -12.56 to 11.98 mmHg로 각각의 환자에서 두 안압계의 안압 차이가 넓게 분포한 것으로 나타났다. 본 연구에서도 골드만압평안압계의 22 mmHg를 기준으로 대상을 나누었을 때 22 mmHg 이상에서의 두 안압계의 상관계수가 22 mmHg 미만에서의 상관계수보다 통계적으로 유의하게 높은 것으로 나타나 유사한 결과를 보였다. Lee et al [18]은 한국인을 대상으로 Icare TA01i와 골드만압평안압계를 비교하였는데, 두 안압계의 안압은 뚜렷한 양의 상관관계를 보였으며(r=0.728, p<0.001) Icare TA01i의 안압이 평균 2.23 mmHg (95% 일치한계, -3.96 to 8.42) 낮게 측정되었다고 하여 본 연구와 비슷한 결과를 보였다. Kim et al [19]은 한국인에서 Icare PRO를 골드만압평안압계와 비교하였는데 Icare PRO의 안압이 평균 1.92 mmHg 높게 측정되었으며, 95% 일치 한계는 -4.52 to 8.37 mmHg였다. 이 연구에서는 안압이 30 mmHg 미만인 환자만을 대상으로 하였는데, 두 안압계의 안압 사이 상관계수(r)는 0.700 (p<0.001)으로 본 연구에서 안압이 22 mmHg 미만인 환자에서의 상관계수 0.671과 유사한 결과를 보였다. 최근 Badakere et al [14]은 156안을 대상으로 본 연구에서와 동일하게 Icare ic200을 골드만압평안압계와 비교하였는데 Icare ic200의 안압이 평균 1.27 mmHg 높게 나타나 본 연구와 상반된 결과를 보였으며, 이들의 연구에서는 골드만압평안압계의 안압이 1 mmHg 높아질 때마다 Icare ic200의 안압은 1.04 mmHg 높아진다고 하였다(p<0.001). 본 연구의 대상들에서 평균 연령이 더 높고(55.3 vs. 64.5세), 골드만압평안압계의 평균 안압이 더 낮은 점(19.5 vs. 16.7 mmHg) 등이 연구 결과의 차이에 일부 영향을 미쳤을 것으로 보인다. 이외에도 본 연구에서 중심각막 두께가 더 두껍고(517.6 vs. 538.9 μm), 안축장이 더 긴 것(23.3 vs. 24.2 mm)과 함께 한국인을 대상으로 했다는 점이 연구 결과의 차이에 영향을 끼쳤을 것으로 예상된다. 한국인을 대상으로 했던 Lee et al [18]의 연구에서도 동일한 모델인 Icare TA01i와 골드만압평안압계를 비교했던 외국의 여러 연구 결과들과[24-26] 상반되게 Icare TA01i의 안압이 낮게 측정되었는데 이를 설명할 수 있는 요인을 제시하지는 못 하였다. 리바운드안압계의 여러 모델이 서로 다른 인종에서 유의한 측정치의 차이를 보이는지는 향후 인종 간 비교 연구를 통한 확인이 필요할 것으로 보인다.
이전 연구들에서는 중심각막두께가 리바운드안압계와 골드만압평안압계의 안압 차이에 영향을 준다는 결과도 있고[11,18,20,21] 반대로 영향을 미치지 않는다는 보고도 있다[14,17,19,22,23]. 본 연구에서는 중심각막두께가 두 안압계의 안압 차이와 유의한 연관성을 보이지 않았다. 본 연구에서 나이가 많은 것이 두 안압계의 안압 차이가 커지는 요인으로 나타났는데, Tonnu et al [27]은 나이가 듦에 따른 각막의 경직도가 안압 측정에 영향을 미칠 가능성을 제시한 바 있으며, 골드만압평안압계와 같이 각막의 넓은 면적에 접촉하여 안압을 측정하는 안압계에 비해 접촉 면적이 좁은 안압계는 각막 경직도에 영향을 덜 받을 것으로 기대했다. 본 연구에서는 각막곡률이 클수록 두 안압계의 안압 차이가 유의하게 커지는 것으로 나타났으며, 평균 각막곡률 ±2 standard deviation에 해당하는 40.46-47.70 diopters (D)의 환자만을 대상으로 분석한 경우에도 통계적 유의성이 유지되었다(β=0.268, p=0.026). 골드만압평안압계의 안압은 각막곡률이 3 D 증가할 때마다 안압이 1 mmHg씩 높게 측정되는 것으로 알려져 있다[28]. 따라서, 리바운드안압계가 각막곡률의 영향을 덜 받는다고 가정하면 각막곡률이 높은 경우에는 두 안압계의 차이가 증가할 것으로 예상해볼 수 있다.
본 연구의 제한점으로는 연구가 후향적으로 진행된 점을 들 수 있다. 대상 환자들은 본원 안과를 정기적으로 방문하는 환자들이며, 생체계측치는 이전에 검사된 결과들을 전자의무기록에서 조사하였다. 하지만, 동일한 검사 방법과 순서에 따라 두 가지 안압계를 이용하여 안압을 측정하였으며, 두 가지 안압계의 검사를 각각 한 명씩의 독립된 검사자가 진행하여 검사의 신뢰성을 유지하고자 노력하였다.
본 연구는 넓은 범위의 안압과 생체계측치를 가진 한국인을 대상으로 최근 소개된 Icare ic200과 골드만압평안압계의 안압을 비교했다는 데에 의의가 있다. 결론적으로 Icare ic200의 안압은 골드만압평안압계의 안압과 좋은 일치도와 22 mmHg 이상의 고안압에 대한 높은 진단능을 보였으나 안압이 평균 3 mmHg 낮게 측정되었다. 따라서 Icare ic200은 임상에서 골드만압평안압계를 대체해서 사용하기보다는 고안압에 대한 선별검사로써 유용하게 사용될 수 있을 것으로 보이며, 협조도가 낮은 환자에서 제한적으로 골드만압평안압계를 대체하여 사용할 수 있을 것으로 보인다.

At 15.65 mmHg on the Icare ic200, sensitivity and specificity were 90.9% and 83.9%, respectively. At 17.80 mmHg, sensitivity and specificity were 84.8% and 94.6%, and at 18.05 mmHg, sensitivity and specificity were 81.8% and 95.0%, respectively.
